    Compassionate staff, strong therapy, concerns

I found the staff overwhelmingly caring, compassionate, and dedicated - residents seemed happy, engaged in activities, and the facility offers strong therapy, a chapel, and excellent meals. However, I also experienced serious operational problems: missed or late medications, unanswered call lights/phone calls, cleanliness issues (including bed-bug reports), and disorganized or unresponsive management. Overall it's a warm, home-like place with great people, but I have safety and leadership concerns and would recommend with reservations.

    About Belleview Care Center

Belleview Care Center sits at 1616 Weisenborn Road in Saint Joseph, MO, and has 90 certified beds, but most days about 81 residents live there, and it serves as both a nursing home and assisted living center, plus it has memory care and independent living options so elders get care that matches their needs, whether they're recovering from a hospital stay, need daily help, or live with dementia or Alzheimer's. The facility is a for-profit corporation controlled by Weisenborn Road Consulting LLC and connected to Providence Health & Services and Vertical Health Services through a series of owner companies, with no big awards or recognitions listed at this point. Belleview offers a mix of care, from skilled nursing and memory support to independent living apartments, and has medical services like wound care, diabetes help, medication management, therapy (physical, occupational, speech), mental health support, hospice care, and even podiatry, and you'll see features like three daily meals, snacks, dining rooms, and kitchenettes in some living areas. Memory care programs include secure settings and staff trained to support folks with dementia and Alzheimer's, while assisted and independent living units come in studios, one-bedroom, or two-bedroom apartments, and there are private and semi-private rooms in the nursing facility, with more affordable independent options at $1,500 to $4,500 per month and nursing-type care costing between $5,000 and $7,000 a month. Staff offer help with bathing, dressing, grooming, personal care, medication, and moving around, and most staff speak English, though some can help in other languages if needed, and residents can use amenities like a fitness center, salon, activities and games rooms, cable TV, housekeeping, laundry, WiFi, and guest parking. Safety features include sprinkler systems and accessible rooms for handicapped residents, and the facility offers transportation for appointments or outings, plus different activities for socializing, crafts, or health programs. Belleview Care Center is rated by Medicare.gov, has no recent awards, but does have a long history of inspection deficiencies-recent federal inspection reports show problems with infection control, nutrition standards, such as not always serving food from approved suppliers or in ways that match professional rules, plus a February 2025 inspection listed 12 deficiencies, and in March 2025 there was a complaint report showing 1 more deficiency, adding up to 61 documented deficiencies, with two tied to infections and at least one related to protections against abuse, neglect, or exploitation with the potential for more than minimal harm. The facility's licensed nurses provide about 3.06 hours per resident per day and help assess every new resident to make a plan that fits their needs, and the staff encourage families to talk openly about costs, contracts, licenses, inspection results, and staff-to-resident ratios before making any decisions. Availability varies each day, and Belleview sometimes pauses new admissions, so it's necessary for interested families to call about up-to-date openings. Those who move in can expect personalized care plans and support from skilled staff, but families should also be aware of the facility's inspection and compliance history, which is public, and should ask questions about infection control, safety, meal planning, and the steps in place for resident protection.
