Brooke Haven Healthcare sits at 1410 Kentucky Avenue in West Plains, Missouri, and folks will find it's a family-owned place where seniors can get different kinds of care like skilled nursing, short-term rehab, long-term care, hospice, memory care, and even palliative care, so you see they've got their bases covered whether someone's staying for a little while or needs a lasting home and what folks seem to notice is that residents can pick between private rooms, with eight set aside, or share semi-private rooms if they want that, and everyone can use the comfortable living spaces or go outside when weather allows to visit or relax. The facility's licensed, Medicare and Medicaid certified, and has a dedicated post-acute rehab center-there's a special entrance and nursing station for folks coming in after a hospital stay and looking to recover before heading home, and that center also provides outpatient rehab if someone only needs help for a short time without moving in, and they don't forget about things like physical, occupational, speech, or even respiratory therapies, since the rehab specialists spend a lot of time with residents depending on what's needed. Brooke Haven keeps 24-hour licensed nursing staff on-site too, and they've got a physician who leads care plans and checks on everyone like clockwork, plus a dietitian to give advice about meals and nutrition, and if someone needs medications, the staff works with an outside pharmacy that sorts and packages everything to keep it all straight. People will see hair care and manicures offered at the on-site beauty and barber salon too, plus activities like arts and crafts, group birthday parties, and all kinds of social or exercise programs so residents and families can stay busy and social if they want, which sometimes makes a big difference in how connected folks feel. They offer transportation for appointments and errands that can be set up ahead of time. The laundry's handled for everybody, and there's an active resident action council, letting folks speak up about their wants and needs, which can make things feel a bit more like home. The facility also supports companion care and respite stays if caregivers outside the facility need a break, and they seem to focus on treating everyone as an individual, involving families, and giving attention to community health with a whole range of services under one roof. Holly Osgood is the administrator, so there's a point person leading all the staff and services, and Brooke Haven shares info on Facebook for folks looking to keep in touch with what's happening or find more details about their programs.

Caring staff, leadership needs improvement
I found the staff overwhelmingly kind, professional and attentive, the building clean, pleasantly decorated, parking convenient, and meals generally good - rehab/24-7 care helped my loved one. However, I also noticed recurring issues (occasional urine smell, inconsistent meals) and troubling management failures: staffing shortages, missed baths, a resident left on the floor, a COVID death in their care, and an unresponsive administration. Staff truly do their best and many residents seem happy and well cared for, but leadership and accountability need serious improvement. Mixed feelings - great day-to-day care from staff, proceed with caution.
