    Understaffed facility, unsafe, slow improvements

I had a mixed but mostly disappointing experience. There were genuinely caring, hardworking CNAs and new Cottonwood management has begun an impressive turnaround, but chronic understaffing and poor communication left residents unsafe: long delays for assistance and bathing, missed oxygen, bedsores, filthy rooms/wards and odors, missing clothing, loss of independence, and rude business-office/staff attitudes. My MPOA and visitors were sometimes restricted, spiritual visits blocked, and police/APS/hospice had to be involved - improvements are happening, but I still wouldn't trust this facility with a loved one.

    About Billings Rehab & Nursing

Billings Rehab & Nursing sits in a quiet area of Billings, Montana and serves seniors needing skilled nursing and rehabilitation care, though right now they aren't taking new residents. The facility accepts Medicaid and Medicare, and runs as a for-profit limited liability company with Brandy Lake as the administrator. The place holds up to 100 certified beds, and usually has about 70 residents each day. The staff includes professional caregivers who help with daily needs like bathing, dressing, transferring, and medication management, and they also provide 12 to 16 hour nursing care. There's a 24-hour supervision and alerts to keep people safe, with help for both wheelchair users and those who aren't able to walk. Residents moving in get coordination support, and the staff includes people who speak English and other languages.

The facility offers a range of rooms, both private and semi-private, with private showers, big screen TVs, and furnished spaces. Housekeeping, laundry, and dry cleaning services help keep things tidy. Residents can join community-sponsored and resident-run activities, arts projects, game nights, movies, and gardening. There are walking paths, a game room, and an arts room, plus plenty of chances to socialize. The dining room is open all day and serves meals to match special dietary needs, including meals for diabetes or allergies. The kitchen prepares food on site, and there's help for those who need it with feeding or meal planning.

Billings Rehab & Nursing provides transportation and has parking, so residents can get to appointments or get out when needed. They also work to offer a warm, home-like atmosphere with inclusive programs, trying to support physical, emotional, and social wellness through different activities and therapy programs. The team aims for a safe and tranquil space and says they want to improve all the time for both residents and staff.

It's important to note that recent state inspections found a number of issues at this facility. There have been 60 deficiencies in the last reports, covering things like protection from abuse, neglect, and exploitation, as well as resident rights and nutrition standards-such as food safety and storage. Two separate infection control issues were reported. Billings Rehab & Nursing has a nurse turnover rate of 75.5%, which is above the Montana state average, and a lower-than-average staffing level, with 2.87 nurse hours per resident per day compared to the state's 4.0 hour average. Because of its history of serious quality concerns, government agencies have marked it with a special focus designation. The facility does have a website for more information at www.billingsrehab.com.
