The Pines at Blue Hill sits at 414 N Willson St in Blue Hill, Nebraska, and has 62 certified beds for skilled nursing care, so you see quite a few people coming and going every day, some for short-term stays and some for longer times, and the staff, who use their own employee portal, is managed by Avid Healthcare Group, focusing on growing wellness rooted in compassion, which means they try to keep folks comfortable and moving forward in their own way, and the place has different types of care, including personal care to help with dignity and choice, memory support, subacute care, and therapies-physical, occupational, and speech-offered by professionals who stay on site, and there's always a nurse on duty, though the facility clocks about 3.58 nurse hours per resident each day, and the nurse turnover rate is 70.7%, which seems high if you ask me, plus they've had 39 total deficiencies and 6 related to infections in inspections, so you might want to ask about how they handle these things. There are both private bedrooms and twin beds in shared rooms, and you'll find amenities for comfort like a residents' lounge, walk-in tubs with safety rails and built-in chair lifts, beauty barber services, laundry, and religious services, and three types of meals are served every day. The building itself has features that help with safety and accessibility, and the grounds have a nice front porch swing and gardens with blooming flowers and decorations, plus a shared outdoor area for enjoying the fresh air. Folks can join in on social and activity programs, and there's case management to help with different needs, and amenities in the city mean you can use the Keno, ball fields, library, aquatic center, golf course, fitness center, and senior center nearby if you're able to get out. They also have 24-hour nurse coverage, housekeeping, and some infection control efforts tied to dietary services. The Pines at Blue Hill tries to match services to residents' needs, with options for short-term rehab or longer-term stays, aiming to keep everyone as healthy and independent as they can, and they work with the Nebraska Health Care Association, though there's no specific info on management structure or how exactly employees are managed, so you might want to ask if you visit.

Therapy excellent; watch meds/hygiene closely
I'm grateful the staff are loving and therapy, activities, and food helped my mother regain strength and stay emotionally supported while they work toward discharge home. However, I'm alarmed by missed medications, short staffing, and dirty bathrooms (feces present) - the basic care and cleanliness need serious improvement. I recommend cautiously: excellent therapy and kind staff, but monitor meds and hygiene closely.
