St. Luke Health Services sits on East River Road in Oswego, and the people living there get to see the Oswego River from the windows while taking part in daily life in a nonprofit nursing home that's been around for over 30 years, offering both long-term care and short-stay rehab, so if someone needs help after an injury or operation, they can get physical, occupational, or speech therapy in a unit made for short stays, usually no more than 90 days, and those who plan to stay longer get skilled nursing and a wide range of support every day, with caring staff helping with things like medication, wound care, IV antibiotic therapy, and personal care needs like laundry, meals, and companionship. The staff includes nurses trained to handle complex care, including palliative care for those near the end of life through the Comfort Care Program, and also support for Alzheimer's and dementia with specialized programs. St. Luke keeps people active and social with a busy events calendar that includes scheduled shopping trips, spiritual activities, group talk show watching, educational sessions, fitness activities, and chances for everyone to be involved in the community. There's a medical model adult day health care program too, meant for folks who still live at home but need daily health checks, case management, and therapies like dental care, nutrition help, and regular health monitoring, and if someone needs care at home after a hospital stay or for a long-term health problem, the Home Health Services can send staff out to help keep them safe and comfortable. The community focuses on building personalized care plans that try to honor each person's dignity, help them keep as much independence as possible, and support the families as they make decisions, and you'll see people helping each other, building friendships, and joining activities if they choose. St. Luke Health Services belongs to The St. Luke Family of Caring, serving the greater Oswego County area, and the campus includes programs like Pope's Commons, Bishop's Commons, and St. Francis Commons, each designed to meet different care needs for adults, whether someone needs assisted living, adult day health care, or a place for ongoing skilled nursing, all with the same focus on safety and living life to the fullest for over 300 people every day, and with caregiving resources to give families guidance through all the changes that come with aging and long-term health care.

Caring staff, but serious concerns
I had a mixed experience. The therapists, nurses, CNAs and social worker were caring, helpful with rehab/PT, family-friendly, active and attentive - the facility is attractive, generally clean, and the meals decent. However I also saw serious problems: poor communication, missed notifications, hygiene/scabies incidents, medication/dosing and staffing lapses that led to injuries and safety concerns, plus chaotic billing. Use cautiously - good for rehab when staff is engaged, but insist on clear communication and close oversight if you consider long-term care.
