    Good therapy, pervasive neglect, unsafe

    I had a mixed but mostly negative experience. Therapy and rehab staff were excellent and helped with mobility and social engagement, and a few people (Ruby, Danelle, Theresa, Hannah) were compassionate and attentive. However I also saw pervasive neglect: ignored calls, no phone in the room, messages unanswered, patients left in wheelchairs for hours, long med delays, yelling/abusive aides, filthy rooms, unsafe equipment, security lapses and poor communication. I cannot recommend this facility without major changes and I plan to file a complaint.

The staff includes skilled and supportive professionals who offer services focused on individual needs, working as a large team to help every resident reach their best possible well-being, physically, emotionally, and psychosocially, and residents can expect medical care, nursing care, rehabilitation, social services, and clinical care tailored to them. Colonial Park has specialized care units for things like dementia, post-surgical orthopedic care, hip and joint replacement recovery, amputee training, tracheostomy care, wound care, infection management, respiratory support, and many others, with 24-hour nursing care and a full-time physician available, and the staff receive special training, for example, in CPR and wander guard safety.

    The center has amenities such as free phone and cable TV, an electronic security system, private and semi-private rooms, laundry, a business office, religious services, beauty and barber services, hospital transportation, and a concierge service, and meals get planned by executive chefs to meet a range of dietary needs and still taste pretty good. Colonial Park uses electronic medical records for accuracy and speed, and there's a wander guard platform that helps keep residents safe by monitoring for risk of wandering, escalation of alarms, and asset tracking. Rehabilitation options include physical, occupational, speech, and recreational therapy, with help for pain, stroke recovery, cardiac care, mobility, and memory support, and residents get access to social work and counseling services as needed. They also provide specialized programs like nutritional counseling, IV therapy, pharmacy service, podiatry, dental, ophthalmology, and lab work.

    Colonial Park reports 3.09 nurse hours per resident per day, and has a nurse turnover rate of 52.6%. Inspections have resulted in 33 deficiencies since the last period, including two infection-related ones. The most recent complaint report was filed on November 5, 2024, with one deficiency noted. The last standard inspection from July 22, 2024, found eleven deficiencies, including one related to infection control. Other cited deficiencies include not always honoring residents' rights to voice grievances without fear of reprisal, and not always providing treatment or care strictly according to physician orders or resident wishes; inspectors rated these as posing potential for more than minimal harm, though found no actual harm in these cases. Colonial Park says its mission is to respect and partner with residents and families about treatment choices, food, and activities, and aims to promote comfort, dignity, safety, and support through personalized care.
