Staten Island Care Center sits in Staten Island, New York, and has 300 certified beds, usually seeing about 281 residents a day, which means the place is pretty busy but there's still room, and it's been open for more than 25 years as a skilled nursing facility, though folks there say it's been a nonprofit nursing and convalescent home for over 40 years before it became for-profit, and now it's owned by Constance Leifer and Joel Leifer, with Yitzchok Wiener managing since April 2012. The center belongs to the Excelsior Care Group, and while the nurses there give 2.71 hours per resident every day, which is less than the New York average of 3.7 hours, the place has a nurse turnover rate of 24.3%, lower than the state's average, so it seems like nurses tend to stay a bit longer here than lots of other places. Staten Island Care Center holds a five-star Quality Measures rating from the Centers for Medicare & Medicaid Services, though it has had 10 inspection deficiencies in total, including 2 infection-related issues, and these include things like resident rights, assessment, care planning, quality of life, and activities, which means the inspectors look at how the staff handles daily living and rights, and the facility works on giving activities to meet all residents' different needs, so there's always something going on, whether it's Oktoberfest or celebrating health care food service week for dietary workers. Folks can take part in activities, have recreational therapy, or even join educational or therapeutic programs, and there's memory care along with therapies like speech, language, respiratory, occupational, and physical, so that means people with many different needs can get help here. The staff offers diet counseling, pharmacy work, dentists, and social workers, with transportation options using bus routes S40, S44, and SIM30, plus 24-hour emergency care and skilled nursing services, so help is close by at any time, and if someone needs a doctor, physician services are there too. The center is set up for both short-term rehab after a hospital stay and long-term care, and it has special programs for things like orthopedic rehab and cardiac care, and there's also subacute rehabilitation, so folks coming in after surgeries or heart problems can recover. The building has a gallery, photo and video galleries, and families can even take a virtual tour, which is helpful, and there's a newsletter for updates on events and facility news. With a non-discrimination policy, a pandemic emergency plan, and an emphasis on infection control with handwashing and advanced cleaning, the staff tries to keep everyone as safe as possible. Staten Island Care Center is a for-profit limited liability company, but it's approved by the New York Department of Human Services and Department of Public Welfare, and it's even affiliated with groups like the National Association of Private Schools for Exceptional Children, the Council for Exceptional Children, and the Autism Society of America, so care covers a wide range of physical and educational needs, and the whole facility seems to focus on wellness, quality outcomes, and the comfort of its residents, all while making sure people feel welcome and supported in their daily lives.

Strong rehab care, weak nights
I had a deeply mixed experience. Daytime staff, rehab therapists and many aides were kind, professional and went above and beyond - they helped my husband regain mobility and even felt like they saved a life. But nights and some nurse shifts were inconsistent and understaffed: long call-bell waits, residents left soaked in urine, infrequent bedding changes, poor room cleaning and occasional supply shortages. Food was often cold, repetitive and unappetizing. I witnessed coordination breakdowns, ignored requests and at least one fall and delayed response that felt like neglect. If you can rely on strong family oversight, the rehab and daytime teams are excellent; without that oversight I would not recommend it.
