Carolina Rivers Nursing and Rehabilitation Center sits at 1839 Onslow Drive in Jacksonville, North Carolina, and offers both short-term and long-term skilled nursing care for seniors who need 24-hour support. The facility cares for people after surgery, injury, or illness with a strong team that handles everything from medication management to wound care, cardiac and pulmonary care, diabetic support, and stroke or neurological care. The staff provides physical, occupational, and speech therapy alongside high acuity care, incontinence care, non-ambulatory care, and hospice care, so people with chronic health problems get the attention they need. There's memory care for those living with dementia or Alzheimer's, tailored by an interdisciplinary team that develops personalized plans focused on dignity, safety, and keeping independence as much as possible. Carolina Rivers puts effort into making the environment safe and welcoming, with safety steps in place like regular screenings, cleaning with electrostatic methods, HEPA filtration, and mask use for visitors and staff, all to try and cut down the risk of illness. Meals here come through a nutrition program with balanced vitamins and minerals and are meant to actually taste good, which is important for folks who don't always like what's served in hospitals. The center features private and semiprivate rooms, a barbershop on site, indoor and community gathering spaces, and activity programs both on and off the property-sometimes with devotional activities if people want that. The facility brings in dentists, social workers, optometrists, registered dietitians, and covers intravenous therapy, psychological care, and general personal care for different ages. Medicare and Medicaid have certified Carolina Rivers, and its parent company, Britthaven, runs other centers in North Carolina, Kentucky, and Virginia. The center isn't taking new residents right now, but you can call about future openings. Online reviews show mixed opinions, with a current rating of 2.7 out of 5 based on 18 reviews. The staff speaks English, and they might have workers who know other languages too. They aim to keep folks as safe and comfortable as possible, while encouraging social activity and supporting both residents and families through different health and emotional challenges, from memory loss to serious illness.

Filthy facility with neglectful care
I placed my dad here for rehab and my experience was mostly awful. The place smelled of smoke, urine and rotten food, rooms and furniture were filthy, and I saw roaches and peeling walls. Call lights and meds were often ignored or delayed, residents left in waste, bedsores and safety scares occurred, and management was unresponsive about care and billing - we removed him. A few nurses, CNAs and therapists were kind and skilled and did excellent work, but they were the exception. I would not recommend this facility given the hygiene, safety and staffing failures.
