    Understaffed mismanaged facility; unsafe care

I placed my mother here for rehab. The building is old but clean, with nice outdoor spaces and many genuinely kind, friendly staff who know residents by name and run activities - when staffing allows. That said, the facility is severely understaffed and mismanaged: long nurse-call delays (I personally saw ~35 minutes), medication delays of days, poor handoffs/continuity, delayed or discontinued PT for insurance reasons, and weak medical oversight. My mother suffered dehydration and a hospital readmission; there were pressure ulcers, unexplained bruising, and other safety incidents. Administration was often rude, visiting rules were strict, and billing/insurance handling was aggressive and confusing. Staff try hard with limited resources, but I would not leave a non-mobile or medically complex loved one here without constant family advocacy.

    About PruittHealth - Crystal Coast

PruittHealth - Sealevel is a medium-sized nursing facility with 104 beds that provides both short-term rehabilitation and long-term care services. Operating under corporate, for-profit ownership, the care home accepts both Medicare and Medicaid, making it accessible to residents with a variety of financial situations. It is not part of a continuing care retirement community, focusing its services on individuals needing short or long-term skilled nursing rather than offering a full spectrum of senior living options.

Residents seeking short-term rehabilitation at PruittHealth - Sealevel can expect an environment focused on recovery following hospital stays related to strokes, heart attacks, infections, or accidental injuries. Approximately 45.4% of residents are able to return home after discharge, reflecting the facility's commitment to improving residents' functional abilities. Notably, the percentage of falls resulting in major injuries is reported as 0.0%, and 7.7% of those staying for short-term rehabilitation have experienced infections requiring hospitalization, while 13.7% required emergency room visits during their stay. To support patient care, nursing staff provide an average of 2 hours and 55 minutes of direct care per resident each day.

For long-term care residents, PruittHealth - Sealevel provides assistance with a range of daily activities and medical needs, such as administering medications, dressing, eating, and personal hygiene. The care home places an emphasis on helping individuals maintain their independence, with 82.2% of residents retaining the ability to manage activities of daily living without extensive assistance. Emergency room visits for this group average 1.8 per 1,000 patient days. The administration is attentive to preventative care, as shown by a 98.6% flu vaccination rate among residents. Staffing levels are consistent with those of the short-term rehabilitation unit, though weekend staffing is rated below average for maintaining the same ratios as weekdays.

PruittHealth - Sealevel also undergoes regular health, fire, and safety inspections conducted by national regulatory bodies to ensure a safe and compliant living environment. The most recent inspection metrics reflect its adherence to national fire protection standards, with periodic reviews helping to maintain quality and safety standards. Over the last three years, the facility has paid fines totaling $9,463, indicative of their responsiveness to regulatory feedback.

Residents and families may find comfort in the knowledge that PruittHealth - Sealevel is committed to providing care in a structured setting where daily medical and personal needs are met. Through participation in federal programs like Medicare and Medicaid, comprehensive care is accessible to a wide range of seniors in need of skilled nursing services, whether their stay is for short-term rehabilitation or ongoing long-term support.
