    Supportive staff, great rehab, inconsistent

I appreciated the warm, family-like staff, clean/homey atmosphere, nutritious meals, and excellent PT/OT and activities that helped my loved one. Many caregivers were attentive, compassionate, and made us feel supported. However, staffing and communication were inconsistent - I saw delayed assistance, charting/medication lapses, and occasional hygiene/safety issues that required family advocacy. Overall a mixed experience: great people and rehab, but stay vigilant and be prepared to speak up.

About Wilson Healthcare and Rehabilitation Center

Wilson Healthcare and Rehabilitation Center, also known as Brian Center Health and Rehab Wilson, sits at 2501 Downing Street SW in Wilson, North Carolina, and you'll find it run by a for-profit limited liability company linked to Simcha Hyman and Naftali Zanziper under YAD Healthcare. The place holds up to 99 beds, with 81 of those certified, and it usually has skilled nursing, both long-term and short-term rehabilitation, and a sub-acute unit meant to help people recover after surgery or illness, with the goal of cutting down on trips back to the hospital and infections. The staff includes a wound nurse alongside folks trained in speech, physical, and occupational therapy, working to help residents get back their abilities and keep as much independence as possible, and they set up care plans to fit what each person needs, with programs for memory care, cardiac care, pain management, renal care, orthopedic rehab, cancer care, hospice and palliative care, and dysphagia therapy, all aimed at helping the physical, mental, and social wellbeing of the people who live there, and you'll usually hear English spoken by the staff.

They've been cited for two deficiencies, neither of which caused actual harm but had the potential for more than minimal harm-one has to do with how they get, store, and serve food and needing to stick better to the rules, and the other is about providing proper care for folks with problems controlling their bladder or bowels or with catheters, needing to keep up prevention for infections. You see a nurse turnover rate of about 51.6%, yet they do have a total nurse staffing hour per resident per day at 3.32, and trained staff work under Administrator James Carlone. Folks say it feels compassionate and welcoming, with programs to help people manage pain, recover strength, communicate better, and cope with difficult illnesses or memory loss. Amenities provide the safety and comfort expected from a skilled nursing place, with support around the clock.

The center's not taking new residents right now, but it has features like downloadable vCard for contact details and options for private tours when openings come up. Most people think of it as a straightforward health care and rehab center, driven by a mission to help folks do as well as they can, backed by professional standards, and ability to care for some complex needs, keeping things clean and safe and always aiming for kindness.
