    Good short-term, poor long-term trust

    I had a mixed experience at this small Tulsa facility. The building was clean, newly decorated, secure with a pleasant courtyard; staff were generally compassionate, knowledgeable and attentive, therapy was good, food/serviceable, and there were many activities and on-site services. However rooms are tiny and often shared with no private bathroom; I saw or heard about hygiene lapses (bad smells, soiled diapers), improper wound care, unexplained falls/bruising, inconsistent long-term nursing, and poor communication/billing and occasional rude management. Overall I'd consider it for short-term rehab but would not trust it for long-term care.
