    Neglectful, chaotic care; compassionate staff

I had a horrible experience here - many requests went unfulfilled, staff were rude or lazy, and care was inconsistent with bad odors, a gas smell, hot rooms and not enough assistance (no shower for five days). The nurses, DON, housekeeping and dietician were excellent, and some corporate staff were helpful, but administration and the business office were chaotic - paycheck delays, account errors, time-clock problems and aggressive demands to sign things. My dad died, there were substantiated complaints and a state investigation; I walked out and will never bring someone here. I love and miss the residents, but this place is not family-friendly or fit for anyone to live in.

    About Wewoka Healthcare Center

Wewoka Healthcare Center is a medium-sized, for-profit facility with an 87-bed capacity, providing both short-term rehabilitation and long-term residential care. Operated under an LLC ownership structure, the center accepts both Medicare and Medicaid, ensuring a range of financial options for residents seeking care. It is not part of a continuing care retirement community and does not offer Alzheimer's-specific care within its services. The facility is dedicated to supporting individuals recovering from medical events such as strokes, heart attacks, infections, or accidental injuries, as well as those needing ongoing daily assistance with medical and non-medical needs like medication management, dressing, eating, and mobility.

In recent evaluations, Wewoka Healthcare Center received below-average ratings in both short-term rehabilitation and long-term care. The facility had a short-term rehabilitation rating and a long-term care rating each noted as below average, referencing areas such as the ability of residents to return home, occurrence of falls with major injuries, rates of infections leading to hospitalization, and the frequency of emergency room visits among residents. For short-term rehabilitation, residents spent an average of 3 hours and 2 minutes per day with nursing staff, and 4.7% of them experienced falls resulting in major injuries, while 8.3% required hospitalization due to infections. Emergency room visits occurred for 13.3% of short-term residents during their stays, indicating a higher than ideal rate of acute care needs during rehabilitation.

Long-term residents at Wewoka Healthcare Center received the same ratio of nurse staffing time, and the facility was noted for maintaining consistent staffing levels between weekends and weekdays—a point highlighted as excellent. About 69.2% of long-term residents retained the ability to manage self-care activities such as moving, eating, and personal hygiene independently. Flu vaccination uptake among residents stood at 61.0%. Emergency room visits for long-term residents were recorded at 2.6 visits per 1,000 patient days, reflecting ongoing medical attention required by some residents.

Health and safety inspections are conducted at least every 15 months by governing health agencies, monitoring aspects such as food safety, care standards, and the upholding of residents’ rights and dignity. Several inspection findings noted at the center included concerns related to meal palatability, food safety and storage practices, implementation of infection prevention and control programs, ensuring nurse aide performance observation and regular training, and proper execution of psychiatric medication monitoring protocols. Other inspection outcomes highlighted certain shortcomings in the fulfillment of residents' rights regarding dignified existence, self-determination, as well as the maintenance of a safe and hazard-free environment to prevent accidents.

The facility has also undergone follow-up actions to address findings such as support staffing in nutrition services, ensuring call systems work in bath and restroom areas, meal timing flexibility according to residents’ preferences, and timely assessments for mental health and intellectual disabilities. While many of these issues were categorized as presenting a low potential for harm and typically affected only a few or some residents, ongoing evaluation and corrective measures remain essential aspects of maintaining care quality.

Over the last three years, Wewoka Healthcare Center has paid fines in connection to regulatory oversight, underscoring the importance of attention to facility operations and quality improvement. The center continues to provide care services to the community, balancing the needs of residents requiring short-term recovery and long-term support. Both residents and their families rely on the facility to deliver daily care, foster independence where possible, and respond effectively to medical changes during a resident’s stay.
