Rose Haven Nursing Center works as a skilled nursing facility and has 121 beds, providing residential care for disabled and elderly people and focusing on long-term care, memory care, and rehabilitation services, so folks who need help for a short time after a hospital stay can get short-term care, while people who need help every day have options for long-term support or memory care, and they see to it that people with Alzheimer's or dementia get the help they need from trained staff. The center runs around the clock with 24-hour nursing care, and their care team includes nurses and therapists for physical, speech, and occupational therapy, along with specialty services like wound care, bariatric care, IV antibiotics, catheter care, respite care, help with tube feeding, and diabetes management, and all these services show their commitment to a high standard, since they've got state certifications and are a member of the Oregon Health Care Association. Rose Haven has well-kept, renovated spaces with gardens and outdoor areas, so folks can relax or spend time with visitors, and the place gives attention to comfort and a home-like feeling, so the staff aim to help everyone live with as much dignity and independence as possible. Residents get living spaces tailored to their needs, including private rooms for those who want them, and everyone has a personalized care plan with help available for daily activities, rehabilitation, and managing medication, and the facility also offers help with nutrition, laundry, and housekeeping. The staff includes on-site healthcare providers, and there's an in-house pharmacy and lab, so medical support is close at hand, with licensed services covering discharge planning and care coordination too, and transportation to medical appointments when needed. The community puts work into diversity, equity, and inclusion, and creates programs for social interaction, building a community where people can take part in activities and relax outdoors, and there's extra support for caregivers too, making sure people feel valued, safe, and cared for by dedicated staff. Folks who need hospice or palliative care find symptom management and emotional support here, with the focus always on comfort and good communication. Rose Haven Nursing Center always tries to promote well-being, keeping close attention to safety and emotional health while supporting residents in a simple, respectful, and caring community.

Caring staff, systemic safety issues
I found many staff to be warm, professional, and compassionate-Nikki and Sam were favorites, and Cherish and Karen communicated well-rehab was excellent and some team members went the extra mile. However, the facility is clearly understaffed and overworked, with medication-administration gaps, slow or unresponsive call lights, rude/unprofessional behavior at times, and real safety concerns (falls, choking hazards, forced care). The building is old, run-down, sometimes smelly, and needs renovation; administration and record-keeping were unreliable. In short: wonderful people doing their best, but systemic failures make me hesitant to recommend it.
