Umpqua Valley Nursing & Rehabilitation Center sits in Roseburg, Oregon, and has been around since 2003, offering short-term rehab and long-term care for older adults or anyone needing nursing care, and while it has 118 certified beds, most days you'll find about 78 residents living there, which means it doesn't feel crowded. The place is affiliated with Volare Health and provides a wide mix of services, including skilled nursing, physical therapy, occupational therapy, speech therapy, wound care, help with nutrition and diabetes, and support for those who need catheter care, IV antibiotics, hospice, palliative care, and even bariatric care. Regular amenities include private rooms for folks coming out of hospital stays, an on-site pharmacy and lab, laundry and housekeeping, recreation, and help getting to doctor's appointments, plus staff are said to be friendly and helpful, making sure people get care based on their own needs, and there's also 24/7 supervision and special safety steps in place for the residents who need them. Even though they do try to use good healthcare standards and new practices, recent state inspections found some problems, like in May 2024 when a report listed 23 areas that could be better, with 3 of those tied to infection control, and over time, inspectors have marked 4 deficiencies related to infection and a shortage in nursing and doctor staff because there wasn't always enough or a licensed nurse on duty for every shift. Staff turnover rates for nurses are a bit higher than the Oregon average, with 52.9% leaving the job yearly compared to the state's 50.9%, but the nurse to resident staffing hours per day is above average at 5.30 hours, which means people often get more attention from nurses than in some other places. Care at Umpqua Valley focuses on helping people gain independence after surgery or illness, keeping up their dignity, and giving support for chronic illnesses, and the services on offer could help most seniors or folks with complicated health needs, whether they need help for a little while or a long while, plus all residents, no matter their situation, get access to therapy services, recreational programs, help with everyday activities and things like meals, medication, and personal care with a goal to keep everyone as comfortable and supported as possible.

Compassionate staff, clean facility, inconsistent
I had a largely positive experience: bright, clean facility with high-quality meals and a cheerful atmosphere, and exceptional, compassionate staff - nurses, CNAs and the rehab/PT/speech teams helped my recovery and were attentive. The dietary and kitchen staff were wonderful and administration usually addressed concerns, but care was uneven at times - corporate staffing shortages, poor communication, lost clothing/phone issues and occasional neglectary lapses were real problems. Overall I would recommend this facility for rehab and kind caregivers, but advise staying vigilant about communication and belongings.
