Broad Mountain Health & Rehabilitation Center sits at 500 W Laurel Street in Frackville, Pennsylvania, and offers a range of care for older adults, including skilled nursing, long-term care, memory care, assisted living, personal care, and independent living, and you'll find home care services mentioned in their overview, too, so folks looking for different levels of help all in one place can find various options here, and they work hard on inpatient rehabilitation and post-acute care, especially after surgery or a heart or stroke event, with speech, occupational, physical, and cardiac therapy from registered nurses, therapy aides, and medical professionals-though they run as a for-profit facility and belong to the Saber Healthcare Group, which operates other communities throughout the region. Residents get nutritional counseling and personalized care plans focused on helping them reach their best ability, and people going through stroke recovery or needing cardiac therapy get specialized attention, whether they're staying for a short rehab or longer-term nursing care, and the staff speaks English with some able to help in other languages, so communication shouldn't be too much of a challenge. The community has studio and suite rooms to choose from, focuses on a home-like atmosphere with community amenities set up for safety and comfort, and it's the only senior living facility right in Frackville, about 6.7 miles from Pottsville, which means choices in town are limited, and, despite scoring the highest among local facilities with a 7.4 out of 10 average rating, the Center holds a 1-star rating from the Centers for Medicare & Medicaid Services due to inspection history, as past government reviews have varied in their grades. Broad Mountain accepts Medicare and Medicaid, but there's currently a waitlist for new residents, so they aren't taking newcomers right now, and families should know there's no resident or family council at this time. The Center's website offers more details on senior care options and the facility often runs at a high occupancy, about 80%. Staff use an interdisciplinary team approach, and every resident gets an individualized therapy plan, whether the need is short-term rehab, long-term nursing, or help with memory issues, but it always comes with a focus on supporting daily life and basic health needs above everything else.

Caring staff but inconsistent management
I placed my mom here and overall I feel she's well cared for: the frontline staff and nurses are warm, attentive, keep us informed, rooms are very clean, the grounds and entry are nice, the food and therapy are excellent, and activities (bingo/community events) really help. Rebecca in admissions and social services were especially helpful. My concerns: chronic understaffing and inconsistent management - I've seen late meds, slow responses, unprofessional supervisors, and some staff morale issues. Despite that, the caring staff made a real difference in my mom's recovery, so I recommend with reservations.
