    Compassionate staff; homey facility; concern

I had a largely excellent, 5-star experience - compassionate, family-like staff who went above and beyond, a clean, well-maintained, homey building, engaging activities and strong rehab/CNA instruction (Julie Ann Arron was exceptional). Staff treated residents like family and kept a positive, caring atmosphere. My main concern: I observed substance use among residents and learned complaints were filed with the state with little management action - that needs to be addressed.

About Pickett Care and Rehabilitation Center

Pickett Care and Rehabilitation Center sits in a quiet neighborhood of Byrdstown, Tennessee, close to Dale Hollow Lake, and offers 69 beds for seniors needing different levels of help, whether short-term respite care, rehabilitation, or long-term skilled nursing care, so you'll probably find it's small enough for staff to know the residents' needs but still has room for specialized help and activities. Managed by Signature Healthcare since 2007, this for-profit facility says it gives the warmth and comfort of a home and has a family-based organizational culture, focusing on treating everyone with dignity and respect. Staff follow a resident-centered approach, planning each person's care based on individual needs, wants, and even religious or cultural backgrounds, which shows up through their care navigation and quality of life programs, and they even offer interfaith spirituality activities for those interested. The center's membership in the Eden Alternative Registry and the Eden Registry's recognition means they try to do more than just standard care, moving toward person-directed support so people can keep their independence and sense of self as much as possible.

Pickett Care reports an average of 54 residents each day and employs nurses with a turnover rate lower than the state average, and those nurses are around for 3.81 hours per resident each day, which is almost at the Tennessee average, and that should help with getting more personal attention. The place is a member of the Tennessee Health Care Association and TNCAL and offers CNA training programs, so some staff may be learning on the job but others have experience and qualifications. Pickett also tries to keep patients active and involved with life enrichment programs, and their rehabilitation services support both physical and emotional health, making sure not to overlook folks' spiritual needs or cultural backgrounds.

Recent inspection history does show some trouble though-fifteen total deficiencies show up in their reports, including infection control, protecting residents' belongings, and keeping families informed, but state inspectors said no actual harm occurred, and management seems aware that changes need to be made to meet federal standards. The most recent standard inspection happened more than two years ago, even though these are usually supposed to happen every 9 to 15 months, and the record shows complaints, with two deficiencies found from a February 2025 complaint. Owners include several holding companies and private individuals, so the organization is not locally owned.

People looking for care at Pickett Care and Rehabilitation Center will find a small community with a range of services, a staff-to-resident ratio that helps with personal care, and a push towards resident-focused care plans, but inspection history and ongoing management mean it's wise to check for the latest reports and think carefully about the environment.
