Magnolia Healthcare and Rehabilitation is a for-profit nursing home in Columbia, Tennessee, that offers skilled nursing, long-term care, short-term respite, and rehabilitation services and serves an average of 92 residents each day with 181 certified beds available, and has between 51 and 200 employees working under a Limited Liability Company. The staff spends an average of 3.75 nurse hours per resident each day, but there's a high nurse turnover rate of 84.2%, which is much higher than the state average of 49.6%. The place focuses on making rooms clean and updated, especially with renovated areas like a newly improved subacute unit, and they serve made-to-order breakfast to help residents feel at home. Magnolia puts effort into offering varied clinical and quality of life programs including interfaith spirituality, and tries to respect different cultures and backgrounds. Staff are often described as friendly, wonderful people who help each other out and support the residents, and some say the administration team is helpful and quick to respond. Inspections are supposed to happen every 9 to 15 months, but right now, inspections have run late. The latest inspection showed three infection-related issues, including citations for failing to control infection (F0880) and problems making sure residents' rights are honored, like the right to refuse treatment or make an advance directive (F0578). People mention the facility is usually clean and residents seem to be treated like family, and while Magnolia states a commitment to a safe, caring environment and giving residents as much independence as possible, it has also received a range of health inspection reports that should be considered. Amenities and clinical offerings aim to keep residents as comfortable as possible, and rehabilitation is an important part of their care for physical and emotional wellbeing, and the management group is known for being responsive. Overall, Magnolia Healthcare and Rehabilitation gets a 3.5-star rating from 11 reviews, and while there are positive remarks about the staff and renovations, potential concerns about staff turnover and inspection findings also stand out.

Beautiful facility, inconsistent resident care
I had mixed experiences. The building is beautifully updated, welcoming, and many staff are kind, professional, and treat residents like family - rehab, therapy and meals were often excellent and the location is convenient. However, care was inconsistent: I saw long call-light delays, missed meds and antibiotics, poor wound care/bedsores, soiled linens, short-staffing and spotty communication that led to hospital trips. Management can be helpful at times but not reliably - I'd recommend with caution depending on the unit and staff on duty.
