Lexington Post Acute sits at 727 E Church St in Lexington, Tennessee, and stays open day and night, always there every day of the week, which means you'll find someone around whatever hour you need. This place brings together many kinds of care in one spot, so you can start out living on your own in an independent living area where folks can have a home-like feel, only with peers nearby and help available if you want it, but then they've also got assisted living, which means staff can lend a hand with things like getting dressed, keeping track of medicine, eating, and making sure you're safe. There are Board and Care Homes for just a handful of people-so it stays small and personal-and these places have a daily routine made to make things a little easier on aging folks. If life gets more complicated, Lexington Post Acute hosts skilled nursing where nurses give round-the-clock medical care, help with wounds, and help people bounce back after an illness or surgery, and the staff here includes people named James, Stephanie L NP, Jones, Cary, Pagoaga, Jennifer C MSNE, and Edwards, Kara, all working to support health needs. There's a community for memory care, where the doors stay watched and the team uses special programs and memory-lifting activities for those with dementia or Alzheimer's, and they promise a safe space where people can find calm even when memory starts to slip, with staff all day and all night. The people here say they want everyone to feel at home and put together care plans that fit the person, not just the illness, and that means offering social gatherings and resort-like comforts so active seniors don't have to fuss with chores. They cover a lot of ground with levels of care and always say they want to make things comfortable, safe, and positive. They keep a website at lexington-pa.com that lists directions and extra details about what's offered, but the main thing is, if you need care for a short stay to recover or a long haul to keep steady, they'll have both a place and a plan.

Warm programs but dangerous neglect
I had a mixed, emotional stay. The rehab program, attentive caregivers (especially Entertainment Coordinator Mrs. Viki), excellent activities, and an exceptionally clean, comfortable facility made my mom feel at home. But serious problems - poor responsiveness, missed promised caregiving visits, very low food quality, inconsistent staff and turnover - culminated in alarming neglect (feces on a resident, severe dehydration and critically low blood pressure that led to hospitalization) and safety concerns about unsecured access. I appreciate the warmth and programs, but the safety and care failures are unacceptable.
