    Caring staff but emergency delays

I appreciated the caring, patient staff (Tamara was very helpful) and saw my mom improve with effective PT/OT; rooms are clean and roomy, meals are generous if a bit bland, and the quiet, friendly atmosphere with activities and transportation felt like good value. However, we had two incidents that required 911 and staff response was delayed - a serious safety concern. Overall I recommend the facility but advise asking about emergency response protocols.

About VanAyer Senior Living and Rehabilitation

VanAyer Senior Living & Rehab sits at 460 Hannings Lane in Martin, Tennessee, and serves as a nursing home, senior living, and rehabilitation facility with a variety of care options for older adults, so you'll find everything from independent living and assisted living to memory care, skilled nursing, home care, and long-term care. The building's got 91 certified beds and usually houses about 66 residents a day, with a nurse staffing level of 3.96 hours per resident, which is better than the state average, and a nurse turnover rate of 38.9%-below the usual rate in Tennessee. Rocky Top Healthcare LLC owns the place and The Ensign Group's got its name on the paperwork, while Betty Swindell's been the administrator since January 2025; the facility's for-profit and not part of a CCRC, but does connect to American Health Communities and the Tennessee Health Care Association.

Residents can get help with daily routines like dressing, transfers, bathing, and meal prep, and there's a 24-hour call system and plenty of private rooms with bathrooms, kitchenettes, air conditioning, Wi-Fi, and cable TV. The community's set up for folks who need lots of care, so there's skilled nursing, hands-on memory care, and intermediate care for frailer adults who depend on others, and for anyone who's between a hospital stay and home, there's short-term rehab. Nursing services handle physical assessments, respiratory care including tracheostomies, wound care, IV therapy, pain management, diabetic and ostomy care, and detailed medication management. Vocational therapists offer speech, physical, and occupational therapy on-site, while home health services, palliative care, and hospice care round out the list of medical support. A CNA training program helps with staff education, and employees are trained in dementia care, social work, and property upkeep, so the team should know how to cover a lot of problems that can come up.

VanAyer's got community programs with activity rooms, daily events, arts and music, a movie theater, and plenty of outdoor gardens and walking paths, not to mention access to a library, fitness room, laundry, and a computer center, and residents can get transportation for errands or appointments. Meals are served in a dining room with a chef who can prepare meals for diabetics or those with allergies, so there's usually something safe to eat, but you'll want to know that the facility's received deficiency citations in past inspections-including F0812 for nutrition issues and F0695 for care quality; problems in the inspection history highlight concerns with food sourcing and respiratory care, and there's also been a longer-than-normal gap between regular inspections, which are supposed to happen every 9-15 months.

For those needing help with finances, VanAyer accepts both Medicaid and Medicare, and through Golden Cross Senior Ministry, offers some community supports like financial help and activities for residents who need extra support or face homelessness, which means there's more than just basic services on offer. The facility doesn't allow smoking and offers plenty of social, mental health, and educational resources-including seminars and access to industry events like the THCA/TNCAL Convention. The community keeps residents connected, not only through scheduled activities and outings but also by making it easy for families and seniors to find resources or help through its resource center, career programs, and facility finder.

VanAyer holds a 3-star rating from the Centers for Medicare & Medicaid Services, and although it's got many amenities and a broad assortment of programs for nursing, health, and long-term care, anyone considering this place should also review inspection reports and past deficiencies, because ongoing quality oversight's important. The rooms offer a choice of layouts, and the staff supports daily living for those who need extra hands, whether it's non-ambulatory residents requiring frequent help or someone wanting independent living. VanAyer's built for all levels of need, from respite care and companionship to advanced, round-the-clock skilled nursing, and it aims to provide a safe and supportive place for older adults in Weakley County.
