    Beautiful facility but inconsistent care

I placed my mother here nearly three years ago and the building and services impressed me - beautiful therapy room, bright activity and dining areas with big windows, very clean rooms equipped for high-acuity care and respirators. Many staff (Becky, Ashley, LPN Sheeteah Blair among them) were five-star: compassionate, attentive, quick to respond, recovery-focused and family-like - wounds healed and we often felt safe. However care has been inconsistent: infrequent check-ins, poor communication, no help with meals or showers at times, a COVID episode with weight loss, apathetic/long-tenured staff, and high daily costs. Mixed overall - excellent hands-on caregivers and facility, but expect to stay involved and monitor care closely.

    About West Meade Place

The HealthCare Center at West Meade Place sits over in West Nashville at 1000 St. Luke Drive, holding up to 120 residents and providing both nursing care and assisted living services, so you'll find both skilled rehabilitation and long-term care available here, with a special 14-bed orthopedic unit for folks coming from the hospital, each in private rooms, which people tend to appreciate for the privacy when they're recovering. The place has private and semi-private rooms, all with 24-hour nursing help from experienced staff, and a team that includes physicians, nurses, and therapists aiming to give each person a plan that fits exactly what they need, working together using a flexible, interdisciplinary approach, and there's a whole lot of options when it comes to medical support, like wound care, IV antibiotics, and medication management-plus skilled therapy services that range from physical, occupational, and speech therapy to pain rehabilitation, stroke care, and even low vision rehab. Meals get planned out by a dietitian and chef, so people's nutritional needs are covered, and the dining room has some impressive views-something people tend to notice. They've also got a state-of-the-art therapy center, a big atrium, and a garden area for getting outside when the weather's good, which helps lift spirits. There's a unit focused on respiratory care, even for patients needing ventilators or tracheostomy weaning, and in-house dialysis is available for those who need it. Activity programs are regular, including games, exercises, outings, and classes for creative expression, and extra help gets given for daily tasks like bathing, eating, walking, grooming, and more, along with medication supervision for safety. Hospice care is another service here, focused on comfort and dignity, and there's a clinical social worker to help residents and families with transitions or resources. West Meade Place is a privately owned facility that contracts with the VA and is a member of the Tennessee Health Care Association, so it's connected to community resources, trainings, and events, and even runs a Certified Nursing Assistant program. Reviews of the place currently average at 4.6 out of 5, with folks often mentioning the warm, professional approach and a strong commitment to personal care and rehabilitation, so while no place fits everyone, this one aims to give comprehensive care and support for seniors in a comfortable, caring environment.
