    Beautiful building; excellent therapy; inconsistent

My experience was mixed. The building is beautiful, very clean, pet-friendly and therapy (PT/OT/rehab) was excellent, and several nurses, CNAs and new leadership were genuinely caring and proactive. However care is inconsistent - I saw frequent medication delays/errors, safety concerns like wounds/dehydration, lost belongings reported, and chronic understaffing with long waits. Dining was a weakness: meals were often late, poor quality and kitchen staff could be rude. I'd recommend with caution: visit, vet med management and staffing, and avoid if your loved one needs high-level Alzheimer's or constant assistance.

About RollingBrook Rehabilitation and Healthcare Center

RollingBrook Rehabilitation and Healthcare Center sits at 750 Rollingbrook Drive in Baytown, Texas, and it does what you'd expect from a good skilled nursing facility, covering both rehabilitation and general healthcare needs, so people come here for short-term rehab or long-term care depending on what's going on with their health. The place has 130 certified beds and sees about 89 residents on a typical day, housing people in semi-private suites or private rooms with showers, temperature control, WiFi, phones, flat-screen TVs, cable, and electric beds. They keep things cool with air conditioning, and folks can get around in wheelchairs throughout the building, which is laid out to be home-like so families and residents can spend time together comfortably; there are even family social areas and a beauty/barber shop on-site. The whole place is pretty accessible, with parking for visitors, and there are outside spots like a park-like landscape, walking paths with slopes and stairs for therapy, sitting areas, a gazebo, and a fountain out front, making it a bit nicer for anyone who likes getting outdoors.

RollingBrook's staff pay attention to individual needs, focusing on patient-centered care with help from skilled nurses, aides, and therapists, though you should know the nurse turnover rate's fairly high at 75.8% compared to the Texas average, and that's something to think about if you worry about seeing new faces often, though the staff nurse hours per resident per day come out to 3.56 which is higher than the state average. They run all sorts of services here - from basic healthcare and skilled nursing to more specialized care options, helping with recovery for those who need rehab between a hospital stay and heading back home. For fun or just to pass the time, residents get daily social activities, and the housekeeping crew keeps things tidy each day without skipping. There's a gym for therapy and a gallery for residents to enjoy, and there are amenities for exercise and rehab with equipment right on hand to help folks regain mobility and independence, sometimes after an illness or injury.

Management control here's been with Sharlyn Threadgill and Rollingbrook Rehab LLC since May 2021, and the place has ties with Momentum Skilled Services and is owned by West Wharton County Hospital District. Inspection reports from December 2023 and January 2025 flagged some deficiencies, especially related to nutrition and dietary practices, but no actual harm was reported, though the total count hit sixteen documented deficiencies, so that's something to be aware of if you're looking for complete transparency. The center's clearly focused on keeping people moving and regaining their abilities, and every resident gets their own plan made to match their needs, whether it's a short stay to recover or long-term support, all in a setting people can settle into, with some nice touches like daily activities, barbershop, and outdoor walks if they want to enjoy a little sun or chat with family.
