Brenham Nursing and Rehabilitation Center sits at 400 East Sayles Street in Brenham, Texas, and you'll find it serves Washington County and is a skilled nursing facility with space for 128 certified beds and most days it has about 117 residents. The center provides both short-term rehab and long-term care, offering a range of help like physical, speech, and occupational therapy, plus special programs for wound care and memory care. Residents get both private and semiprivate rooms with full baths, and there are day rooms and lounges where folks gather, along with a therapy gym and even a kitchen to help with practice for going back home. People like the different amenities-WiFi, cable, a beauty salon, barbershop, private dining rooms for special occasions, and transportation for outings and appointments. There's help for people who need cardiac, pulmonary, stroke, respiratory, tube feeding, and even end-of-life hospice or palliative care, and the facility also provides Alzheimer's, Dementia, and Memory Care support. The staff works on personalized care plans and they're there for admissions seven days a week. Home health services are also available to help people regain or keep their independence, and folks can get outpatient therapy or post-surgical and orthopedic care too. It's run as a non-profit and is affiliated with Wellsential Health. The center has earned accreditation from healthcare groups, and the staff aims to maintain high standards, though inspections have found some issues in recent months, including infection control problems, incomplete care plans, and making sure resident rights and preferences are respected-things they're working to improve. You'll find nursing care services day and night, and the nurse staffing averages 2.88 hours per resident per day with a nurse turnover of 56 percent. Residents and their families can use Medicare, Medicaid, or other insurance to help with costs. The place tries to focus on dignity, comfort, and keeping a sense of community, and is known locally for its compassion and support, even as it continues to address challenges seen in federal oversight reports.

Compassionate staff, serious safety issues
My experience was mixed. I saw compassionate leadership (Meagan, Alice), many caring nurses, attentive doctors and PT, strong rehab results, a clean/social facility and moments of excellent customer service. However, staffing is inconsistent with high turnover, long response times, poor meals and laundry issues, and there were serious safety concerns reported (theft of belongings, rough handling, falls and an aggressive roommate that worsened dementia). Administration is sometimes responsive, but I'd visit unannounced and reconsider if dementia or safety are major concerns.
