Val Verde Nursing and Rehabilitation Center sits in Del Rio, Texas, and has 76 certified beds and an average daily census of about 52 residents, so it's not the biggest or the smallest place around, and people come here for short-term rehabilitation, long-term care, memory care, or after a hospital stay, with room choices like private suites, semi-private, shared, or companion rooms, many with private bathrooms and kitchenettes that are air conditioned and come with WiFi, cable, and telephone, and they've got a beauty salon, recreation activities, and community rooms, which helps folks feel less isolated, and they do meal prep with an eye for organic food, which some might find comforting. Staff include CNAs, LVNs, RNs, therapists, and other healthcare professionals offering services like skilled nursing, daily care, medication help, post-hospital rehab, wound care, cardiac and diabetes management, IV and tube feeding, speech and physical therapy, and different kinds of therapy like Diathermy, E-Stim, Ultrasound, and VitalStim, plus there's support for Alzheimer's, dementia, mental health needs, respiratory problems, and stroke recovery, and they have a focus on more fragile folks who need a lot of help, with nurse coverage that breaks down to 115 minutes from CNAs, 55 from LVNs, and 28 from RNs each day by resident on average, coming out to about 3.10 nursing hours per day. They have pharmacy and blood services, lab work, diagnostic X-rays, housekeeping, and linen help, and there's a 24-hour call system with staff supervision all day and night for safety, especially in secured areas made for people at risk for wandering. They organize groups and activities for residents and families, and they do provide transportation for doctor's appointments and other important trips, as well as home health for people regaining independence, and hospice for comfort in difficult times. The County Hospital District owns 100% of the facility, and Regency Integrated Health Services and Regency IHS of Val Verde have handled management since April 2022, and it's non-profit, which means all money goes back into care and services, not to shareholders, and they take Medicare, Medicaid, private pay, and managed care for payment, with a 7-day admission process to make things a little easier. Val Verde has a nurse turnover rate near 28%, which is about average, and the latest inspection in May 2024 logged 26 deficiencies, including issues with infection control, pharmacy storage, enough food and fluids, and care planning, so there are things to fix, but the facility scores above average overall and for staffing and health inspections on Medicare's star ratings, though its actual nursing home rating from CMS is only 1 star, so people need to weigh that against its high rating locally, since it's rated the best in Del Rio with 9.4 out of 10 from families. You'll get access to skillful care on a post-acute or long-term basis, with a hands-on approach to individualized plans and care led by the Touchstone Experience team, who aim to coordinate care and get people to their best level of health, but as always, it helps to know about both positive qualities and recent citations when thinking about moving in.

Compassionate care with some concerns
I received compassionate, attentive care from kind, professional nurses and CNAs - excellent rehab, good meals, and engaging activities that made it feel like home. Staff were communicative, accommodating, and I'm grateful; I would recommend and would return. Be aware some areas need refreshing, occasional understaffing and long doctor waits occur, and I've heard of a serious safety concern in the past.
