Park Village Healthcare and Rehabilitation is a modern care center with 150 certified beds in spacious private and semi-private rooms, and the grounds are beautifully landscaped, and there's a focus on comfort and safety in a pleasant environment. The facility has received several citations over time, including for not providing enough care and help with daily living activities, not offering routine or emergency dental care, and missing an infection prevention and control program, and, as of the March 24, 2025, inspection, there were 8 deficiencies reported-including one related to infection control-and in total there have been 42 cited issues, with 4 related to infections.
