    Clean quiet caring staff; recommended

    I placed a loved one here and overall I'm pleased - the facility is very clean, quiet, and feels like home, with caring, attentive nurses/CNAs and staff who often go above and beyond with therapy, activities and paperwork. Meals are generally good, private rooms/baths and a nice, small-town vibe make it pleasant, and administration is usually responsive. A few families reported serious issues (missed care, mishandled belongings, rude or unresponsive office staff), so stay involved - but on balance I would recommend this place.

The staff focuses on giving residents a homelike environment, trying to make living there feel warm and comfortable, and they offer medication management, skilled nursing care, diabetes management, wound care, therapy services like physical, occupational, and speech therapy, and also help people who've had strokes, surgeries, or cardiac problems, plus memory care, in-house dialysis, vent care, and hospice, which is a lot of specialty care under one roof. Residents have daily activities to take part in, laundry and housekeeping included, beauty and barber services, religious services, nutritious meals, and transportation if they need to get to appointments or run errands, and there's a bilingual staff so language isn't usually a barrier since some folks don't speak English as a first language. Great Plains Nursing and Rehabilitation also has certified mental health counselors for people who need emotional support, and you can find in-house dental, podiatry, and optometry services, and if you need to pay or check on loved ones, they've set up digital care and payment portals to make things a bit easier. Veterans can get care, and there's a focus on personalized care plans, so residents get what's needed for their condition without one-size-fits-all treatment. The place is recognized as one of the best nursing homes in Texas by U.S. News & World Report, and it has a CMS Five Star rating, which is something the staff works hard to keep up, and it's also a certified Great Place to Work, which says a little something about how the team gets along. They're open Monday through Friday from 8:30 am to 5:00 pm, but currently, you need to call about openings because they aren't taking new patients at the moment.
