Downtown Health and Rehabilitation Center offers skilled nursing care and supports seniors who need daily medical help, providing both long-term care and short-term rehabilitation services so families can feel a little more at ease knowing there's always trained staff available to assist with medical needs, and the center also offers in-house dialysis and specialized vent care for residents who require these services, which isn't something you see everywhere, and caring staff members put effort into making sure residents feel comfortable and supported, and they're part of the Creative Solutions in Healthcare family network, so they can connect residents to different care options if their needs change over time, and there's a full calendar of activities for residents, including social, educational, and entertaining programs to help people keep their minds sharp, their bodies active, and to encourage building friendships, and the memory care program is designed to support those living with dementia or Alzheimer's, giving them a safe setting and extra attention, and the building itself is made to help people stay as independent as they can while still offering plenty of support when it's needed, and you'll find that the staff are always working to improve how things are run and how care is delivered, so if you want to see the place firsthand, private tours are available, and there's always a way to reach someone from the care team if you have questions or need to talk about anything, and being part of a larger network helps the center stay connected to resources for veterans and other special groups, so while it's not the fanciest or biggest place out there, Downtown Health and Rehabilitation Center works to create a compassionate and comfortable environment where people can feel cared for.

Caring staff but inconsistent safety
I have mixed feelings. I found many staff warm, caring, and helpful - they guided us through Medicaid, provided strong rehab and wound care, made Dad feel at home, and the food and activities were often excellent. But chronic understaffing, inconsistent nursing (missed meds, inadequate bathing), poor COVID protocols/communication, cleanliness and safety lapses - culminating in a loved one dying alone - make this facility hit-or-miss; I'm grateful for many caregivers but cannot fully recommend it without major improvements.
