    Warm staff, concerning care issues

I had mixed experiences: many staff were wonderfully caring, friendly and engaged, the atmosphere felt like family and activities were plentiful in a pleasant facility. However I also saw serious problems - poor cleanliness and odors, horrible food, lost belongings, spotty communication and short-staffing, and even care lapses (dehydration/bedsores, unexplained hospital transfers, rude or unprofessional nurses) - so I can't wholeheartedly recommend it.

    About Crossroads Nursing and Rehabilitation

Crossroads Nursing and Rehabilitation sits at 611 Rose Marie Boulevard in Hearne, Texas, and runs as a nursing home under the Creative Solutions in Healthcare network, where patients and families often know Donna Williams and Administrator Jennilee Rosales for their steady presence, and if you stop by any time of the day or night, you'll notice there's always staff on duty, whether it's registered nurses, LPNs, CNAs, or other medical staff, and that's partly why the facility keeps earning those A grades during state inspections, having only picked up a single minor deficiency with no risk to patient safety, maintaining a building that feels home-like and welcoming for both long-term and short-term stays. Residents get a lot of medical support, with skilled nursing and specialized programs for things like Stroke Recovery, Medication Management, Wound Care, and Inpatient Rehab, plus unique care for those needing vent care, memory care, cardiac therapy, or diabetes management, and if someone needs in-house dialysis, podiatry, speech therapy, dentistry, occupational therapy, or nutritional counseling, those services are all on-site, along with home making and transportation for residents who can't quite get around on their own. Veterans Affairs support exists, and families can also use online Care Portals to check on loved ones or manage payment through a Payment Portal, so staying in the loop seems easier for everyone, and the community also keeps links with the Bryan-College Station Chamber of Commerce for local support. Meals are prepared with nutrition in mind-tailored to each person-and the staff crafts personalized care plans that folks can adjust as needs change, making it easier to move from rehab back home, especially after surgery or illness. The place gets about two-thirds full most of the time, offering a wide range of activities, religious services, beauty and barber care, and simple things like laundry, housekeeping, and transportation, all of which add up to a daily routine built around each resident's needs, and people around town know Crossroads for a caring and compassionate approach to elder care, especially with a skilled team that sticks to high standards that tend to match their accreditations. Since Crossroads accepts both Medicare and Medicaid and operates as a for-profit facility, they cover both financial and medical bases, letting families explore options for both short-term rehab and longer-term stays, including assisted living and respite care. The focus stays squarely on each resident's health and wellness, from memory care for cognitive changes to rehab for those working through recovery, and you can always find ways to contact the staff or get updates remotely, which many families find helpful. Crossroads isn't the largest or flashiest, but year after year, its strong staff hours, comprehensive medical support, and transparent communication keep it solid in the community, as the team works to keep things running smoothly.
