    Longtime resident praises staff, warns

I've lived here 19 years and I love being back - the staff are kind, caring, easy to reach, and offered real emotional support during loss; in my experience this is the best facility in East Texas. Rooms are small and a bit crowded, but there are activities on the calendar. I will say I've seen concerning incidents (falls, poor oral care) so looks can be deceiving.

    About Hemphill Care Center

Hemphill Care Center sits in a small Texas town, providing both short-term rehab and long-term skilled nursing care. The place has 89 certified beds and cares for about 31 residents each day, so folks don't usually feel crowded. Hemphill LTC Partners, Inc. has managed things since January 2023, while Winnie Stowell Hospital District owns the place and oversees what happens there day to day. The staff includes registered nurses, certified nursing assistants, physical therapists, and licensed medical practitioners, and they're known for spending more hours caring for residents than the average Texas facility, which helps when folks need close attention. The building offers private and shared rooms, kitchenettes, private bathrooms, and is fully furnished; it's air-conditioned and has phone, cable, and internet hookups. Folks can enjoy three home-cooked meals every day from a menu shaped by a licensed dietician, and the kitchen aims to use organic food and fresh ingredients. Cleaning services, linen changes, and laundry help are part of daily life there. There's always staff around for supervision and a 24-hour call system for emergencies, so folks aren't left alone if they fall or need help in the night. Transportation is provided for medical appointments, and someone can help arrange rides where needed.

Skilled nursing stays available around the clock and the place can handle complex needs, like diabetes, skin and wound care, IV therapy, and pulmonary care, plus pain management for folks who need it. Monthly doctor visits happen on-site, and consultant pharmacists look after medications. If folks need physical, occupational, or speech therapy-say, after a stroke or accident-the rehab team works closely with nurses to tailor the treatments. There's a separate, secured unit for people with Alzheimer's or dementia, and safety features are in place to prevent anyone from wandering off or getting lost. Psychiatric care, counseling, and wellness programs are also part of what the staff pays attention to, and there are religious and family activities, as well as daily events like crafts or games.

The center has reported 32 deficiencies in previous inspection reports, including issues about infection control and upholding resident rights and dignity. The most recent team has made efforts to address safety, accident prevention, and infection standards. The facility has its strengths in hands-on skilled care and therapy, but like many nursing homes, it continues to work on quality of life and regulatory compliance. Medicare and Medicaid have certified the place, and it takes several private insurance plans. While it doesn't discriminate by race, gender, religion, or disability, the staff tries to make things comfortable for everyone and keep families involved. Visiting hours run from 7am to 9pm every day, and folks can use the dining area, lounges, and patio. The overall feeling in the facility is practical and straightforward, focusing on meeting both the medical and daily living needs of senior residents while still holding onto small-town values and routines.
