Monument Hill Nursing and Rehabilitation Center in La Grange, Texas, sits at 120 State Loop 92 and offers both short-term and long-term care for seniors who need some extra help, and you'll see they've got a lot of different services and programs packed in there, which is what people notice right away, like memory care for those with Alzheimer's or dementia, plus skilled nursing care with 24-hour licensed staff for folks who require constant attention or specialized clinical services such as cardiac, pulmonary, respiratory therapy, stroke care, wound care, tube feeding, and even high-acuity and non-ambulatory care, and while they're at it they provide diabetic and incontinence services as well, all of which makes it one of the more comprehensive nursing homes around, though you should know the Centers for Medicare & Medicaid Services has given them a 1-star rating out of 5, but the community rating is higher, sitting at about 8.5 out of 10, and folks say there's a strong reputation locally for decent care. The rooms come in private or semi-private options, and every private room has an en suite bathroom and shower, with wheelchair access, and there's studio layouts available for people who want more space to themselves, which can run about $7,143 a month. They have WiFi and cable, and you'll also find a beauty salon, barbershop, and a bistro available, not to mention a therapy gym with a kitchen set up for those practicing home skills before returning home, and they actually help people regain independence at home through home health services if that's what the family is after, sending support out as needed. For those who want to stay involved, residents can take part in on-site activities-lots of recreation programs, devotional groups, and even off-site trips, because transportation is provided and they don't charge extra for rides to appointments, with parking on site for visitors. There's a secure memory care area for those who wander or need more support, plus community spaces inside and outside, like well-kept gardens, patios, and a courtyard, which folks tend to enjoy when the weather allows. Monument Hill admits people every day of the week, and offers physical, occupational, and speech therapies for things like joint replacements or recovery from surgery, so they have a short-term transitional care unit with private rooms for rehab. Respite care is on offer for families needing a break, and if it comes to it they provide hospice and palliative care to support quality of life near the end, trying to keep things comfortable and dignified for both residents and their families, and you'll see people come here for help with all sorts of challenges, from small to large, with individualized care plans that staff work out together with each resident and family. The building itself has seen some recent upgrades to make it easier for folks with limited mobility, with spaces meant to feel welcoming and to let people gather with others for meals or activities, while memory support and clinical care are tailored for each person. Monument Hill Nursing and Rehabilitation Center is operated by Regency Integrated Health Services, LLC, and is a part of the Texas Independence Health Plan, accepting both Medicare and Medicaid, so there're options for families needing help with costs. Tours are welcome for anyone wanting to see the place and meet staff, because the idea is that you get a full look at the environment and the people before making any big decisions.

Clean, compassionate staff with concerns
I'm very pleased overall - the building is clean, well-organized and homelike with a lovely courtyard, friendly van driver, and excellent rehab. The staff were phenomenal: professional, kind CNAs (Candace, Cassandra, Deborah, Michael, Marian), responsive administration (Liz Rodriguez, Antwon Grant), and therapists like Kinsey who went above and beyond - they took great care of my dad. Be aware of occasional understaffing and slow call-button response, and one CNA theft that was only confirmed after security video, but despite that I recommend this facility.
