    Friendly staff but unsafe rundown

I have mixed feelings. The staff - nurses, supportive care teams and especially Danita (administrator/DON) - are friendly, compassionate and built a strong, positive community; admission was fast thanks to hospital connections and residents often seemed happy. The facility itself is old and rundown (no central air/heating, window units, small dining room, cracked rooms, worn furniture), short-staffed and needs remodeling, with pandemic visitor limits that made oversight hard. Most importantly, I observed serious medical lapses - missed meds, malnutrition/severe weight loss, inappropriate medications and even a death - which creates real safety concerns; I don't trust phone reports and advise families to check on residents in person. Despite excellent staff, I would not consider this home for my family.

    About Magnolia Living & Rehabilitation

Magnolia Living & Rehabilitation sits at 1105 N. Magnolia, in Luling, Texas, and offers both living and rehabilitation services for people who need extra support, though you won't find a lot of detailed information about the building itself or its features like utilities, parking, or outdoor spaces anywhere easily, which can make it a bit hard to picture, but what you can know is the place is accredited by the Joint Commission and has a license from The Texas Health & Human Services Commission, so those things matter if you're looking for some peace of mind about standards. The staff here focus on care and compassion and, from what's shared, try to make personalized care plans for each resident based on individual needs, covering physical, emotional, and general wellness through something they call the CARE Program, so there's attention given to the mind, body, and soul, which isn't always the case in every facility like this. They offer skilled nursing care and several kinds of rehab therapies like physical, occupational, and speech therapy, and there's both long-term care for chronic health concerns and short-term rehab for folks recovering from surgery or illness, which can last anywhere from a couple of weeks to a couple of months. Magnolia Living & Rehabilitation also has home healthcare and memory care services, so there's support for memory loss and related issues. The specific look, feel, and inside features of their apartments or common spaces aren't listed, nor can you find information on rent or sale status on Trulia, but they say quality care is the main goal and mention a five-star rating for quality measures. There's a unique focus on making sure legal protections are in place-paying attention to fair treatment in housing and employment, including on things like gender identity, orientation, and how someone pays for care-so the community tries to make people feel welcome in that way even though there aren't many details about facilities or entertainment. Overall, Magnolia Living & Rehabilitation offers several types of healthcare services and says it puts care and compassion first, with accredited quality, though you'll have to reach out directly or visit to get a sense of what daily life looks like.
