    Clean facility but unsafe care

I have mixed feelings. The building is clean and welcoming, rehab/therapy and food were good, and several nurses and staff were attentive and caring. However, staffing and communication were inconsistent - long call-light waits, unresponsive phones, missed meds/medical attention, delayed hospital transfers and even preventable problems (bedsores, infections) were reported. If you go, stay vigilant and stay involved; you may get excellent care, but there are serious lapses to watch for.

About Mid Valley Nursing & Rehabilitation

Mid Valley Nursing & Rehabilitation sits in Mercedes, Texas, and is owned by the Uvalde County Hospital Authority and run by Touchstone Communities, offering both skilled nursing care and assisted living options in a single-story building. The facility takes in residents relying on both Medicare and Medicaid, with a capacity for 123 beds, serving about 90 people every day, and has rooms outfitted with private bathrooms, kitchenettes, air conditioning, cable TV, flat-screen TVs with satellite, telephones, Wi-Fi, and electric beds-some of the rooms even come as semi-private, but many are large and private. Caregivers here speak English and stay available round the clock to help with bathing, dressing, medication management, transfers, non-ambulatory care, and other daily needs, while a 24-hour call system and emergency alert system offer extra safety.

Nursing care ranges between 12 and 16 hours per day, with 24-hour supervision, and nurse staffing is just a bit lower than the state average at 3.18 nurse hours per resident per day. The nurse turnover rate is about 51.3%, which falls right near the state average. Residents have access to physical, occupational, and speech therapy, with rehab services supporting recovery from surgery, illness, or chronic conditions, plus memory care and dementia support for those needing it. The facility runs long-term care, short-term rehabilitation, respite stays, and post-hospital recovery. Personalized care plans help meet each resident's needs.

Mid Valley Nursing & Rehabilitation features a community activities schedule, inviting people to music programs, art, gardening, movies in an on-site theater, fun outings, and other daily activities. The property includes an advanced rehabilitation gym, fitness and spa/wellness rooms, library, game room, activity room, and family dining room, along with walking paths, gardens, outdoor common spaces, courtyards, a gazebo, and covered patios where residents can relax or join outdoor events. Wi-Fi connects throughout the building. There's transportation for doctor visits and facility activities, plus parking, housekeeping, and laundry services, and family services to help visitors stay involved.

Food's prepared with the help of a registered dietitian, with both community dining and in-room options, choices for allergies, and restaurant-style meals. Residents can also use a private family dining room for special occasions. The facility handles medication management and supports all kinds of dietary needs. There's a 4.0 rating from 52 reviews, and the community tries to keep things warm and comfortable for everyone.

Recent state inspections did show some problems, with 24 deficiencies noted, including infection control, proper handling of resident records, and care planning issues, though there was no actual harm found, just the potential for more than minimal harm. The building's well-kept, but these inspection notes mean families may want to keep a close eye on improvement efforts for safety and care standards. The facility lists itself for claiming and favoriting and has a presence on Facebook where you can view more about activities and events.
