    Clean facility, compassionate staff, peace

I placed my mom here and I'm very happy with the care. The brand-new, spotless facility has spacious private rooms with individual thermostats, a large dining area and courtyard, and A+ meals served in generous portions. The admin team, nurses, CNAs, therapists, dietary and housekeeping are friendly, attentive and compassionate - they've gone above and beyond and given our family real peace of mind. I highly recommend it.

    About Skilled Care Of Mexia

Skilled Care Of Mexia is a healthcare facility with a focus on skilled nursing and rehabilitation care, and while people talk about the home-like setting and warm environment created by recent renovations, there's still a big emphasis on expert care and daily living support for the 57 residents you'll usually find here, which means there's 24-hour skilled nursing, therapy for physical, speech, and occupational needs, and a care team who'll work to tailor plans for each resident, sometimes dealing with short-term rehab or more ongoing support, and they've got specialized services for memory care, vent care, and even in-house dialysis, so lots of medical needs can be handled on-site, plus they've got things for Veterans Affairs. Residents get nutritious meals prepared by culinary experts, and there's dietary support if anyone needs special food requirements met, and the daily activities give folks something to do, while beauty and barber services, laundry, housekeeping, and access to religious services round out the basics. There's flexible visitation so families can visit when it works for them, and families can keep up using an online portal, while care plans include things like wound care, pain management, surgery recovery, cardiac illness management, and help for stroke or neurological conditions, not to mention medication management and help with day-to-day needs. Skilled Care Of Mexia offers long-term care, assisted living, respite stays, and rehabilitation, and you'll find common areas built to feel comfortable and home-like. They have 80 certified beds and are managed by Malisa Blake and Gary Blake, with Mexia II Enterprises, LLC, being the managing company since 2022, and the ownership sits with West Wharton County Hospital District. The nurse turnover rate is a bit lower than most places in the state, and daily nurse staffing comes close to state averages, but the facility's also had its share of issues, like 21 total deficiencies noted at inspection, a major complaint on December 31, 2024, that led to three deficiencies and a $15,239 fine, plus some infection control and food-and-fluid concerns reported, and problems with assessment accuracy and care planning. Even with those challenges, the care team says they're always working to improve services, and the facility belongs to the Creative Solutions in Healthcare network, which can help connect people to additional resources if needed. Residents and families get ways to communicate concerns or ask for help through a customer care line and a message form, which adds another layer of support, so people can know they'll be heard if something comes up.
