    Clean facility with inconsistent care

I moved my mom here and overall I'm grateful - the building is clean and welcoming, the dementia unit is excellent, LVNs and many nurses communicated well, rehab and wound-care staff were outstanding, meals are appealing and supplies readily available, and staff often greeted us by name. That said, care can be inconsistent: CNAs were sometimes unhelpful, call lights slow to be answered, and I've heard other families report more serious neglect and administrative concerns, so tour carefully and ask direct questions.

    About Prairie House Living Center

Prairie House Living Center sits as a large care community with 121 certified beds for skilled nursing and rehabilitation, and while there isn't much information about its history or specifics, the facts do show it covers a good range of senior care needs. The place offers skilled nursing with 24-hour supervision and a call system, providing both short-term rehab and long-term residential care, including support for those who need help with daily activities like bathing, dressing, transferring, or medication management. For seniors with memory loss or dementia, there's a secure memory care unit designed to keep them safe and comfortable, with staff who know how to work with memory impairments. The center delivers a full mix of therapies-physical, occupational, and speech therapies-and stands out for using specialized programs like LSVT BIG & LOUD for movement and speech, as well as being a Music & Memory Certified Organization, which helps bring music into residents' daily lives.

Daily living at Prairie House means residents get meals made by professional chefs, and there are options for dining based on allergies or diabetes needs, plus all-day dining, so folks can eat when they're hungry, not just on a schedule. The care team can offer wound care, assistance for non-ambulatory residents, and respite care if short-term help is needed. Seniors also find laundry services, housekeeping, transportation, and family support services. Life here stays active with arts and music programs, wellness classes, fitness spaces, a movie theater, a game room, and even a spa or wellness space. For those who like to be outdoors, they've got a community garden, walking paths, and outdoor activities, helping people stay mentally and physically engaged. Residents can meet up in comfortable common areas like a library or activity room, and private residences include bathrooms, cable TV, kitchenettes, phone service, air conditioning, and high-speed Wi-Fi.

Salon services, meal prep, move-in help, concierge support, and help with special diets are part of daily life. There's a focus on supporting both independent and assisted living lifestyles, and home care services are available for companionship or non-medical needs for those living outside the center. The facility hosts community-sponsored gatherings, and there's a resident council, so folks can share ideas and concerns. Prairie House accepts Medicare and Medicaid, so payment options can be flexible for families. It's managed by Advanced Healthcare Solutions and has received recognition in areas like Best of Senior Living, All Star awards, and honors for activity programs and friendliness. The staff maintains a friendly, caring approach, always aiming to help residents feel comfortable and supported. Overall, the focus stays on maintaining each resident's well-being, offering choices, and trying to make daily life as simple, safe, and comfortable as can be for people with different health needs and interests.
