    Great rehab therapy, inconsistent nursing

My experience was mixed. The therapy team, many nurses and caregivers were warm, skilled and helped major mobility gains - the food and some staff felt like family and leadership was responsive at times. However nursing responsiveness, hygiene and safety were inconsistent: long call-bell delays, missed meds, poor communication about meds/insurance/discharge, unlocked doors and cleanliness/pest issues left us deeply concerned. I'd recommend this place for short-term rehab if you need aggressive PT, but not for a high-risk loved one who needs consistent, attentive skilled nursing.

About Life Care Center of Plano

Life Care Center of Plano, sometimes called Plano Medical Investors, has served as a nursing home since 1990 with a file open date in 1996 and is owned by Life Care of America, which manages over 200 care facilities. The place sits at 3800 W. Park Blvd. in Plano, right next to Medical Center of Plano and a couple of miles from The Heart Hospital Plano, so you get quick access to hospitals if needed, and the facility itself looks after up to 120 people at once, offering both private and semi-private rooms. They have air conditioning, WiFi, cable television, and telephone hookups in the rooms, along with clean restrooms nearby, and you'll see easy parking for visitors. The main focus here is skilled nursing care, available around the clock, so residents get help with things like medication management, wound care, trach care, ostomy care, diabetes, oxygen or suction therapy, and fall prevention. There's also help with daily activities, post-surgical recovery, and long-term care for those who need it, as well as options for respite or short-term stays.

A team of nurses and therapists builds care plans for every person, including intermediate care, inpatient and outpatient rehabilitation, and wound management, and there's always a registered dietitian making sure meals are nutritious. You'll find both indoor and outdoor spots for relaxation, like day rooms and open courtyards, and residents can use a private dining room or enjoy visits in the cozy common areas. The place has beauty and barber salons for regular grooming, laundry and housekeeping services, and pet visitors are allowed. The building's set up for easy wheelchair access everywhere, and all the onsite services aim to be comfortable and friendly. Life Care Center of Plano has an A+ rating from the BBB, though it's not BBB accredited, and it's regulated by the Texas Department of State Health Services. Mr. Matthew Hinckley serves as the executive director, with Mr. Forrest Preston as principal contact and chairman/CEO. The business accepts feedback through online forms and keeps people updated with a Facebook page and a community photo gallery. Recognized by U.S. News & World Report among the 2025 Best Nursing Homes, the facility stays focused on providing skilled nursing and rehabilitation services with attention to compassion, friendliness, and the little details that make daily life smoother. The lowest daily rate is $190.00.
