Paradigm at the Brazos sits at 2127 Preston St in Richmond, TX, and offers many care options for seniors and caregivers, from independent living and assisted living to memory care, long-term skilled nursing, and even hospice care, as well as home care services both medical and non-medical, so folks have a range of choices whether they need daily help, short-term rehab after a hospital stay, or long-term full nursing care, and every type of living arrangement seems covered except there's no information about pets or outdoor amenities like patios or gardens and you won't find parking facilities, either. The place has 56 certified beds and no pets are allowed, so anyone thinking about moving in should keep that in mind, and though room types and some amenities get mentioned, details aren't really spelled out. The site belongs to Paradigm Healthcare, which runs several centers with names like Paradigm At Faith Memorial, Paradigm At First Colony, and so on, across different Texas towns, and they say they want folks to feel at home and aim to empower their care teams with a focus on warmth and making the environment welcoming. Oakbend Medical Center is an affiliate, so medical ties are strong, and they provide support and referral services as part of care packages. Management falls under Aharon Shkop, Ssc Richmond Operating Company LLC, and Brazos Nursing & Rehabilitation, with a history of management changes in recent years, and the location is an apartment property, though there's not much information on things like the interior layout or which utilities come with rents. The staffing report from recent inspections shows a nurse turnover rate of 29.6% and 2.95 nurse hours per resident per day, and during state inspections in both June and November 2023, there were two deficiencies each time, including concerns over bedroom privacy and keeping the nursing area safe and clean. There's no info about internet, satellite TV, or extras like BBQ pits, pools, or group areas, and the community claims to follow federal housing protections, including protections based on orientation and gender identity, but the site doesn't list out extra fees or details about leases. Some of what Paradigm at the Brazos does best is offering different options under one roof, whether someone needs a day service, independent living, skilled nursing, or tailored clinical care and support after hospital stays, which is helpful for families who want everything handled in one spot.

Excellent care; facility needs repair
I received excellent care from skilled, compassionate nurses and a phenomenal PT team - Tamara and discharge nurse Trudy went above and beyond - and therapy helped me make real progress. That said, the facility is in disrepair (cracked floors, rusty bedside tables, sewer smell in bathrooms) and needs major investment. Some staff were unprofessional or uncaring at times (yelling, ignoring requests, not enough food) and residents seemed bored. Overall I'd recommend the clinical care, but only if ownership fixes the facility and staffing issues.
