    Excellent frontline staff, administration failures

I've been here almost two years and the nurses, CNAs and therapy staff are outstanding-caring, attentive, timely with meds, clean facility, good meals, activities and they've genuinely helped my health. That said, management and billing are a major problem: uncaring toward staff, repeated paperwork errors, disputed/exploitative charges, aggressive collections and attorney threats, and even an MRSA infection here that led to a death in our experience. I love the frontline staff but can't recommend the facility until administration and billing practices change.

About Summit Nursing & Rehab of San Augustine

Summit Nursing & Rehab of San Augustine is a Skilled Nursing Facility that supports people needing different levels of care, whether it's for a short recovery after a hospital stay or long-term help with daily living. Nurses and therapists provide round-the-clock care for the very frail, people on ventilators, and those who need IV therapy, wound care, or even psychiatric services, and folks can get speech, physical, and occupational therapy all on-site. There's a Ventilator Program with experienced respiratory therapists and a pulmonary doctor who visits regularly, so people with complex breathing needs get attention, and they've got psychiatry, counseling, and help from consultants like pharmacists and dietitians. The staff says they aim to keep folks as independent and healthy as possible, and the facility's even helped residents through tough times like Hurricane Harvey. They run daily activities, church services, outings, and wellness programs, inviting families for gatherings, and transportation is available for medical appointments and dialysis. Summit Nursing & Rehab of San Augustine is run by Summit LTC Management LLC, a privately held company out of Fort Worth that also runs other places in Texas, and this location has 88 certified beds and usually about 43 residents a day. The staff includes a team dedicated to getting residents better, but recent state surveys show some problems with infection control and keeping the place safe and comfortable, including not having clear smoking policies in place. Their nurse turnover rate sits higher than the Texas average, though nursing hours per resident are slightly above what's typical for the state. Veterans are welcome here since Summit holds a VA contract, and there's a long-term care ombudsman for residents needing extra help with complaints, free of charge. People can expect essential care services like skilled nursing, hospice, restorative care, respiratory programs, and rehab, yet there have been reports from inspectors about environmental and infection-related issues, so that's something to keep in mind if considering this place.
