Las Ventanas de Socorro is a senior living community with 126 certified beds and an average of about 71 residents each day, and the facility is managed by Socorro Health Care LLC and is affiliated with Fundamental Healthcare. There's skilled nursing care available 24 hours a day and the staff includes registered nurses, licensed nurses, physical therapists, occupational therapists, and speech therapists, and they provide both long-term care and short-term rehabilitation, so someone who needs wound care, IV therapy, or hospice care can get that here, and planned admissions are accepted any time, even on holidays, which folks might find helpful if a need comes up suddenly. The facility focuses on rehabilitation to help people regain their independence, and each resident gets a treatment plan set up to help them reach their best level of health, both physically and mentally, and there's case management for Medicare or insurance as well. There's also specialized care for memory issues like Alzheimer's or other dementia in secure environments, and assisted living is available for those who want support but are still active. Amenities are set up to focus on safety and quality of life, which means residents have access to nutritious meals planned and made by chefs, and there are activities that encourage social, physical, and mental engagement, along with chances for relaxation if a person prefers to keep to themselves a bit. Reviewers give high marks for the friendliness and kindness of the staff, mentioning a positive and uplifting atmosphere, and the place has a reputation for thoughtful and attentive care that respects residents' comfort and dignity. There is, though, a nurse staffing level of about 3.44 hours per resident per day but a high nurse turnover rate listed at 58.2%, and there have been some important deficiencies reported by regulators, including problems with infection prevention and control, areas where proper bowel and urinary care weren't provided, and issues with keeping resident information safe. The facility says it's committed to ongoing improvement in these areas, and there are infection control protocols in place that the team is expected to follow. All told, Las Ventanas de Socorro provides many services and support, aiming for a homelike feeling, but some challenges, especially with staffing and regulatory compliance, are part of the picture, just as in many other long-term care facilities.

Beautiful facility, serious safety concerns
I had a mixed experience at Las Ventanas. The building is beautiful, therapy and activities were strong, and several staff (Claudia, Stephanie, Eddie and some RNs/CNAs) were compassionate, professional and attentive. However, chronic understaffing, missed/incorrect medications, poor hygiene, rough handling/bruising, cold/horrible food and unresponsive administration created real safety and neglect concerns. Visit unannounced, ask hard questions, and be very cautious about long-term placement.
