Teague Nursing and Rehabilitation sits in the community as a long-term care facility that's open around the clock, all week, and folks can find a lot of helpful services there, from skilled nursing care to short-term rehab that often lasts less than six weeks for those who come in just to recover. The place offers health monitoring, different kinds of therapy like physical, occupational, and speech therapy both for those staying and those coming in for appointments, and there's also memory care and respite care for families that need a break. Residents get individualized care plans-so each person has services built just for what they need, and that can mean help with daily living, wound care, pain management, diabetic care, intravenous therapy, even pulmonary services and specialized skin care. The staff-who've been doing this for several years and know the community-work hard to create personal connections with residents and their families, and they focus on making daily life both comfortable and welcoming. Rooms include both private and semi-private choices, and there's specialized dietary service, daily activities, transportation for errands or appointments, and family mail delivery so folks can easily keep in touch. The building's been renovated recently, and inside you'll find a front entrance, a living space, a conference room, and a dining area. When someone moves in, the team does a clinical evaluation to see what each resident needs, and clinical interventions are designed to help everyone reach the best outcome, especially when it comes time to plan for safe and timely discharge. All together, the facility's set up to offer a homelike feel, bring comfort, and provide expert nursing and rehabilitation every day.

Understaffed, neglectful, filthy, overpriced care
I found a few amazing, attentive staff who made the move bearable, but overall the facility is severely understaffed and care was neglectful. Rehab was poorly implemented and my condition worsened after inpatient stays; rooms were often filthy (urine on the floor, uncleaned), meals served on styrofoam, and residents' dignity ignored. Billing was outrageous (~$20,000/month), admin threatened to call Adult Protective Services, police were involved, and I was kicked out after repeated falls. A handful of staff were kind and friendly, but the place needs a completely new, larger, better-run nursing home.
