    Mixed care; staff behavior issues

My experience was mixed. The facility is very clean and calm, and clinical care impressed me - admissions, social services, CNAs, the DON, Miranda Wright (LVN) and the therapy team (Kevin, Reniyah) went above and beyond, explained exercises clearly, and helped ensure a safe, speedy recovery; dietary (manager and Suzanne), meal service, laundry and housekeeping were excellent and eliminated odors. However, HR showed a bad attitude, customer service was poor, and some nurses were disrespectful to family (hung up on calls, restricted access); I was concerned my husband wasn't being fed adequately and felt there were communication/neglect issues. I'm grateful for the caring staff who made him feel at home, but the management/staff behavior toward families needs serious improvement.

About Yoakum Nursing and Rehabilitation Center

Yoakum Nursing and Rehabilitation Center sits in Yoakum, Texas and gives care for people who need daily nursing, short-term rehab, memory care, or hospice. The place has 110 certified beds and stays busy with residents coming for several types of care, including skilled nursing, therapy, post-surgery help, and long-term support for conditions like Alzheimer's and dementia. The grounds have a green space and a short walking path, so residents can enjoy some time outside, and there's an outdoor courtyard plus a therapy gym that even includes a kitchen for folks working on life skills before going home. The rooms can be private or semi-private, and the center tries to make them sunny, spacious, and somewhat comfortable for daily living. There's an on-site beauty salon, WiFi, and cable.

The staff includes nurses, therapists, and care aides 24 hours a day, with a nursing director who holds certifications in wound care, IV therapy, and respiratory care. For those needing something specific, the facility offers physical, occupational, and speech therapy, and admits seven days a week. Special secure units help keep residents with dementia or Alzheimer's safe, with separate areas for men and women, and staff provide behavioral health support. Staff also address care for cardiac, pulmonary, respiratory, and stroke recovery, along with wound and tube-feeding care, and will arrange transportation to medical appointments when needed. There's also help for home health and respite care for families who need a break.

People who live here can take part in activities on-site like movie nights, crafts, church services, music, cooking classes, bingo, and even luncheons at restaurants in town, or enjoy live entertainment. Residents get three nutritious meals cooked for them each day. The environment is usually quiet and calm, with religious services for those who're interested.

The operator is Regency Nursing Centers, under Regency IHS of Yoakum LLC and Barbara Clapp since March 2023, and the facility works with Wellsential Health. The center is certified with Medicare and Medicaid, and has a nonprofit status. For transparency, recent inspections have found 17 total deficiencies, some serious, with issues like protecting residents' belongings and infection control being noted-two infection-related problems were reported in recent visits. The nurse turnover rate is 54%, more than the state average, and staffing levels are lower than the state's typical figure.

People interested can visit, see the place for themselves, meet staff and residents, and check out the different living options. Yoakum Nursing and Rehabilitation Center aims to offer expert nursing, therapy, and memory care, with features meant to make life safer and more comfortable for seniors, though as always, families should look closely at ratings, reviews, and inspection results if deciding on long-term care.
