    Caring staff but overall unsafe

I had a mixed but overwhelmingly troubling experience. A few nurses, CNAs and therapists were caring and rehab helped my loved one, but basic care repeatedly failed: delayed nurse response, poor wound and medication management, residents left soiled, missed meals/malnutrition, falls, theft/financial issues, and very poor communication from administration. The building and some staff looked/acted nice at times, but unsafe, unresponsive practices and management lapses mean I cannot recommend this facility without major oversight and fixes.

    About Autumn Care of Norfolk

Autumn Care of Norfolk sits at 1401 Halstead Avenue in Norfolk, VA, and has been caring for seniors since 1995, and it's one of those places people tend to notice because it does a little bit of everything when it comes to senior care, having 120 licensed beds in a building that offers skilled nursing, long-term care, assisted living, memory care, independent living, and even home care for seniors still living at home but needing a helping hand now and then, and you'll find staff who know how to support seniors with dementia, plus there's always someone around with full 24-hour supervision and care, so you know people aren't left by themselves when they need help, and they have all sorts of services like speech therapy, medication help, and even a specialized Pre-registration Program for planning rehab before surgery, which some families find practical, especially when there's a big operation coming up.

Walking around, you'll see the place isn't fancy but feels home-like, with its outdoor area, garden, patio, rehab gym, a beauty and barber shop if residents want a haircut, and there's even a library full of books, guides, and videos, so folks have something to do besides TV, plus you get all kinds of activities-from outings to historic warships or the botanical gardens to baseball games and local cruises-so residents aren't stuck inside day after day and can visit places like the Norfolk Zoo or go help spot dolphins and whales off the Spirit of Norfolk cruise ship, which gets people talking and moving and keeps them active, because staying involved in life is important at any age.

You'll find the dining is a step above, with a focus on making food that's both healthy and good-tasting, and for those who need it, there's careful attention to medical needs, including medication management, coordinated health care, and a personalized care plan that the staff tailors for every resident, and the administrators-Victoria Dumbra and Ms. Ashlei Pease-run a team with 98 staff members, working to make the place run smoothly, and their commitment was noticed when the facility got the American Health Care Association's 2020 Quality Initiative Recognition, which speaks to the consistent effort here rather than any flash or show.

The building is set up with secure areas for those who wander or get confused, so families with loved ones dealing with Alzheimer's can worry less, and lots of programs focus on key areas like financial planning, legal support, transportation, and veterans benefits, so residents and families have somewhere to turn when they need advice, and the facility has ties to a huge resource directory, listing over 26,000 local and national services that families can access if there's a special need, and they're part of the bigger Saber Health network, which means standards and resources get shared across different facilities, but the care team here seems to know the residents by name and aims to make things personal.

There are assisted living services for those who want to keep their independence but need help with bathing or dressing, memory care for residents with dementia, and even options for short-term rehab or respite care, if family caregivers need a break or someone's recovering before going home, and while the facility is a for-profit place owned by Saber Healthcare Group, it tries to deliver care with both expertise and a bit of heart, focusing not only on medical needs but on keeping people socially and mentally involved.

Rooms come as private or semi-private, with daily rates of $221 and $207, respectively, and with about 39,663 patient days counted in 2023, this isn't the biggest place in the region but it keeps busy, and efficiency and care standards are watched with specific indicators so families can have some peace of mind about quality, and with easy access to I-64 and I-264, visiting's not hard, which helps loved ones stay close. For anyone looking for a place with a range of services-medical care, activities, outings, and a thoughtful environment-Autumn Care of Norfolk offers all of that without being overwhelming, trying to make each resident's day a little better in ways big and small.
