Ashland Health Services is a senior living community in Ashland, Wisconsin, offering a range of nursing and rehabilitation care options and you'll find it's connected with Ashland Health & Rehab Ctr, providing care for those who need 24-hour nursing, long-term support, or skilled rehab services, and the staff put attention into nutritious meals made with good ingredients to help promote each resident's health and well-being. The facility has a certified bed count of 117 but averages about 37 residents per day, and services go from assisted living and intermediate care to chronic care and specialty programs like rehabilitation, physical therapy, and occupational therapy-both inpatient and outpatient-and there are speech therapy and memory care services for people with conditions like Alzheimer's, plus wound care, palliative care, and respiratory therapy onsite. Ashland Health Services also supports medical surgical needs, provides activity and wellness therapies, and has its own dedicated therapy gyms, a Transitional Living Center, private rooms, in-house lab testing, in-house X-rays, swallow studies, wound care, a full pharmacy, help with discharge planning, and even an emergency kit, and for those needing extra support, there's home health and hospice care. They accept Medicare, Medicaid, and private insurance, and people can get specialty care through services such as IV fluids, IV antibiotics, PICC management, automatic defibrillator, and help arranging dialysis transportation. The staff includes nurses and therapists and they offer staffing opportunities for CNAs, LVNs, and RNs, and the nurses average about 2.78 hours per resident each day with a nurse turnover rate of 65.1%. The facility's run by North Shore Healthcare LLC since February 2019, overseen by Troy Baumann, Jeffrey Hoehn, and Tammy Kurtz since 2018, and it has had about 20 inspection deficiencies, including 4 relating to infection control like COVID-19 education and vaccine procedures, and another administrative deficiency concerning reporting to CMS, though neither resulted in actual harm but could have caused some. Ashland Health Services serves people from the Bad River, Lac Courte Oreilles, and Red Cliff Tribes, and has branch offices in Ashland, Bayfield, Iron, Price, and Sawyer counties, and supports shift work for staff through a mobile app. The environment feels homely, and there are modern touches but they do aim to create a comfortable space, with amenities and personal support focused on improving daily quality of life, and there's a strong reputation in the community for steady care rather than flash or hype.

Helpful staff but poor maintenance
My experience was mixed but overall negative. Therapists, the social worker, kitchen staff and some nurses were caring and helpful - Alyssa even went above and beyond; activities and meals were good. However the building was often dirty and cluttered, furniture and décor outdated, maintenance neglected, and I witnessed poor care (slow call-light response, an unwashed resident kept in the same clothes, staff yelling, and a missing shaver). Office staff were helpful at times, but I cannot recommend this place - do not send your loved ones here.
