About Azura Memory Care and Assisted Living at Mierow Farm - Elmbrook

Azura Memory Care and Assisted Living at Mierow Farm - Elmbrook has four purposefully designed homes, with two of them-Elmbrook and Mierow-serving residents with early to late-stage dementia and Alzheimer's, and the other two-Broadway and Wirth-providing general assisted living. The community does a lot to keep people safe and comfortable, using 24-hour call systems, round-the-clock supervision, and staff who speak English. The caregivers undergo over 120 hours of dementia-specific training in their first 90 days, and a registered nurse oversees care so people get the help they need, especially those dealing with memory loss. Residents get help with bathing, dressing, transfers, medication, and all the activities of daily life, including non-ambulatory care, with personalized care plans to fit their needs. Team members always try to support independence while keeping everyone secure, using activities, therapies, and schedules designed for social interaction, physical activity, and cognitive engagement, which can include movie nights, resident-run events, and gardening in the community garden. The buildings themselves feature wide hallways and wheelchair-accessible amenities, a preserved historic barn from the old Mierow family farm, and a seasonal farm stand where residents can get involved, along with a dining room, an arts room, furnished rooms, gardens, and walking paths. Chef-prepared meals come with seasonal and always-available options, and special dietary needs are taken seriously. Residents have many opportunities to stay active and connected, with hospice, home health (Medicare-certified), long-term care, adult day services, and non-medical home care all available. The care team has experience supporting behavioral symptoms like wandering, incontinence, sundowning, depression, and anxiety, with a holistic approach that focuses on each person's story using something called the MOSAIC philosophy. Transportation and parking are provided, and housekeeping and laundry keep things running smoothly. The staff focus on promoting dignity and daily moments of joy, always aiming for comfort, engagement, and safety, so people living there can have both support and a sense of belonging.
