    Inconsistent care, okay short-term rehab

    I had mixed experiences at Manorcare East: therapy, many caregivers, and hospice/support staff were excellent and compassionate, and administration could be helpful. However care was inconsistent and often unacceptable - repeated med/treatment delays, ignored call lights, understaffed weekends, unprofessional CNAs/nurses, and serious hygiene/laundry problems (soiled sheets, stained clothes, bad odors); some wound/feeding-tube/oxygen care was inadequate. Food and activities were hit-or-miss; I'd consider them for short-term rehab only, not for long-term or high-acuity care.

The facility always kept a watchful eye with code-validated security and highly secured areas, especially for memory care where wandering could be an issue, and staff looked after daily needs like help with bathing, dressing, medication management, and mobility, plus support for those needing help getting to doctor appointments, with transportation arranged by the community, and a 24-hour call system so residents or their families could reach someone at any time. Crossroads focused on comprehensive care by hiring skilled nurses and therapists available around the clock, giving wound care, rehabilitation, and skilled nursing for complex medical problems, drawing on a holistic model that treated medical, emotional, and physical needs, always aiming to improve quality of life and promote independence wherever possible. There was access to a chapel, a beauty salon or barber shop, a library, reading rooms, and a community social room for gatherings, art classes, or simple companionship, there were outings to local events, wellness centers, and even some gaming or computer rooms, and residents could count on support for both long-term and short-term needs, including respite care for their families or in-home care for folks preferring to stay put, with trained aides helping with companionship, meals, or basic care. For memory care, Crossroads offered secure, specialized programs and activities that tried to support those with Alzheimer's or other dementia, always in an environment meant to prevent confusion and wandering. The nursing home achieved certification by the Centers for Medicare & Medicaid Services and earned national awards for its level of support, but as time went on and numbers dropped, it saw financial struggles and eventually prepared for closure, following state laws and helping move residents to other homes or back with families, transferring most of the staff to another nearby location. Even with a rating of 3.0 from 22 reviews and recognition for caring staff and a friendly culture, Crossroads Care Center's journey has come with its share of challenges and changes, and it's played a big part in serving Green Bay's older adults and their families, offering practical guidance, a glossary of long-term care terms, and news or articles about healthcare changes, always keeping a steady focus on both the daily realities and future needs of aging.
