The Bay at Sheridan Health and Rehabilitation Center sits in Kenosha, Wisconsin, and you'll see right away that they have a welcoming, well-kept place with a caring team of caregivers and clinicians, and while you're walking through the building you'll find people busy with therapy, staff providing meals, and residents enjoying enrichment activities together, and there's always a focus on helping folks regain independence, whether they're staying short-term or for a long haul because they offer skilled nursing, rehabilitation, long-term care, and assisted living all in one place. They provide stroke and orthopedic recovery, advanced wound care, and memory care programs for people living with dementia or Alzheimer's, and you'll notice their therapists, like Amanda from therapy, make physical, occupational, and speech therapy available both for people living inside the center and outpatients from the community. The Bay offers both independent and assisted living services, along with long-term and respite care, so caregivers can get a break, and they've set up evidence-based programs tailored around each resident, supporting people emotionally and physically, with the goal of keeping everyone's quality of life as high as it can be. Private ownership means they focus on personal touches and flexibility when it comes to daily life and care, and their staff aims to create positive outcomes, whether it's helping someone get back on their feet after surgery or supporting seniors who need full-time skilled nursing. They use updated therapy equipment and techniques for rehabilitation, and they work closely with residents and their families to develop individual care plans, which often include plenty of chances for activities and social connection, so people don't feel alone or forgotten. The facility covers a wide range of health and recovery needs, and they're always looking to make their setting a safe, clean, and encouraging place for seniors at different points of life.

Dirty facility, neglectful unsafe care
I placed my loved one here and was appalled: dirty, outdated building (peeling paint, hot hallways, broken AC/beds), unanswered phones, poor communication, missed meds and bandage mismanagement that led to bed sores, delayed pain meds and hospital transfers. Staff turnover is high, many CNAs seemed untrained or distracted while some nurses and therapists were genuinely caring and skilled. Overall I felt the facility was neglectful and unsafe - I would not recommend and believe it needs serious oversight or closure.
