    Lovely facility, serious leadership concerns

I'm very pleased with the convenient, beautiful and very clean facility - big rooms, lovely grounds, lots of activities, awesome food, and kind, knowledgeable staff who make residents feel at home. However I experienced troubling management/staff issues (toxic, cliquey environment, medication errors and improper med handling, pressure to lie, and unclear rent/money practices), so while I recommend the facility for its care and atmosphere, I have serious reservations about leadership and safety.

    About The Autumn Fields

The Autumn Fields is a senior living community that gives long-term care for older adults, both for those who need assisted living and those who need memory care, and you'll see right away there's a steady staff around with a MedPasser and CNAs during the day and night, always someone available when residents need help. The building has 22 licensed beds, and the rooms are large, furnished, and come with private bathrooms that have showers or handicapped options like walk-in whirlpools, so it's comfortable and easy to get around, and there's telephone access in each room. The place has a peaceful feel with nice gardens, walking paths, and plenty of spaces to spend time with others like the community lounge or library, and you'll find the staff takes care of tasks like housekeeping, laundry, dry cleaning, plus weekly linen and personal laundry without extra fuss. You'll find safety features too, like a full sprinkler system, an emergency alert in every suite, and 24-hour supervision, so families get peace of mind. There's always something happening-a daily activity schedule, movie nights, art in the arts room, regular happy hours, spiritual programs, resident outings or day trips, and support groups for caregivers or Alzheimer's seminars, which draws a lot of folks to come together.

Dining is simple and thoughtful, with restaurant-style food all day, special diets like for allergies or diabetes, three meals daily, fresh fruit always out in the dining area, snacks at 2 p.m., and homecooked items-there's even a cook on staff and meal planners to make sure food's taken care of. The Autumn Fields helps with all the details that matter, like bathing, dressing, transfers, eating, managing medications very closely, taking care of compression gear, and supporting diabetic care with insulin help if it's needed, and there's a registered nurse on site for nursing care. Each resident has their own care plan, built according to a nurse's assessment, and levels of care match the person's needs, whether that's non-memory or memory support, covering everything from daily basics to wound or hospice care, so people do get the exact help they need, without having to ask over and over.

The community's especially mindful for those living with dementia-secured areas, memory care services, and activities planned to be safe, including support for those with mild memory issues, and assistance always nearby. Folks can use services from partner providers like doctors, therapists, lab tests, podiatry, beautician and barber care, and there are in-room visits for those who can't get out easy, which is a relief. The place doesn't allow pets, but you do get transportation to stores, doctors, and events. There's a mix of rooms: studios, one-bedrooms, semi-private, and even two-bedroom options, all with living and bedroom spaces so it's a true home. Respite care's available for short stays after an operation or if family has to travel, which gives everyone a break. The staff helps new residents move in, guides them as they adjust, and makes sure things run smooth day and night-laundry, meals, medical appointments, and daily needs-so you end up in a place focused on providing a safe, friendly, and clean environment with as much independence as possible, lots of ways to stay active, and staff who know their job and do it right, with no fuss or bother.
