    Excellent daytime care, poor nights

    I had a mixed stay. By day it felt warm and welcoming - staff, CNAs and therapists were caring and skilled (special shout-outs to Janet, William and social worker Marissa), therapy helped me walk again, activities were enjoyable, and the food and social events improved since my last visit. However nights were a problem: understaffed shifts, some uncaring night nurses, very long call-light waits, limited or no activities after 3-4 pm and on weekends/holidays, and lingering hallway smells and cleanliness/laundry issues. I also experienced poor communication, small semi-private rooms, security and medication/emergency concerns (slow responses reported), and the place is expensive. Bottom line: excellent therapy and many kind staff by day, but serious staffing, safety and hygiene issues at night - visit and ask detailed questions before committing.

    Madison Health and Rehabilitation Center sits at 110 Belmont Road in Madison, Wisconsin, and has been providing care for seniors for over 30 years, offering a mix of assisted living, independent living, memory care, and skilled nursing care in a licensed building with 90 beds, although only 83 are certified, and they're currently not taking new patients. The community is found within a Continuing Care Retirement Community, so if someone needs more care over time, they're able to stay in the same place while moving between different types of support, which covers everything from rehabilitation after an illness or surgery to more steady long-term care or specialized dementia care, including treatment for Alzheimer's disease. There's an emphasis on healthy meals, activities, and scheduled programs, like fitness and movie nights, to keep residents active and involved, and both community-sponsored and resident-run events help provide some social engagement if folks want that kind of thing. The facility takes Medicare and Medicaid, so folks using those types of insurance are able to get care here, and care staff speak English, with some also fluent in other languages.

    Now, the center is licensed by the State of Wisconsin and is managed by people and companies with ties to Champion Care, and it follows the caregiver-to-senior ratios and safety inspections required by law, but it's worth noting there are some serious quality concerns flagged by the government, since Madison Health and Rehabilitation Center holds a Special Focus Facility designation because of a history of quality issues, including 156 deficiencies in inspection reports, with 22 from March 2025 alone, and out of those, 8 deal with controlling infections and 2 note that residents were put in immediate danger, one involving poor pressure ulcer care and another about bed rails being installed or maintained improperly. The daily census averages 64 residents, with nurse staffing at 3.42 hours per resident each day, which is below the state average, and the nurse turnover rate sits much higher than the state average, at 69.4%.
