Dycora Transitional Health & Living is a healthcare company that helps people move from hospital care to home. The main location is at 5935 South Emerson Avenue, Suite 100, Indianapolis, IN 46237, but there are several other locations in California, including Porterville, Tulare, Reedley, Shafter, Fresno, Fowler, Stockton, and Galt. Dycora mainly offers short-term skilled nursing and rehabilitation care for those recovering from illness, surgery, or injury, and they have both private and semi-private rooms available at their skilled nursing facilities, which are licensed and offer around 112 beds, with about 40 certified beds available as of June 2025. The staff focuses on patient-centered care, providing help with bathing, dressing, medication management, and therapy services like physical, occupational, and speech therapy. The facility also manages care and coordinates transitions for patients returning home after a hospital stay. Residents get 24-hour supervision, non-ambulatory care, an emergency alert system, and nursing support for 12-16 hours each day, with a nurse on call 24 hours. Financial assistance is available through Medicare and Medicaid, and there are community Sservices like food preparation, special diets, meal service, and help with cleaning and laundry. Amenities in the community include a library, arts room, game room, spa and wellness room, movie theater, outdoor walking paths, outdoor community spaces, fitness and wellness programs, music and movie nights, transportation, parking, cable TV, kitchenette, private bathroom, phone, air conditioning, Wi-Fi and internet, plus support for families. Families and residents have councils to address concerns, and there's a focus on creating a supportive environment for both residents and employees. The company started in 2016 and operates on a for-profit basis, with a mission to bring strength and hope to everyone involved. The campus is less than a mile from Watertown Regional Medical Center and near various healthcare providers. Dycora is part of a Continuing Care Retirement Community, which means multiple care options are available, including skilled nursing and rehabilitation. The setup is simple but covers most medical and daily living needs for people who need extra care after being hospitalized.

Successful rehab, but poor communication
I placed a loved one here for rehab and the therapy succeeded, and a few staff and the café were excellent. But I'm constantly frustrated by awful communication: calls go unanswered or get hung up, shifts don't relay information, and I must actively advocate every day. I've encountered rude/unprofessional staff, inconsistent basic care, weekend safety/supply gaps, no social worker, poor cuisine and ambiance, and dated facilities (though generally clean). Overall I can't recommend it - 1 star.
