      Neglectful care leads to death

I placed my mom at this out-of-town facility and was very disappointed. The house is attractive with nice outdoor spaces, natural light and some spacious private rooms, but the atmosphere felt depressing, residents seemed sad, couches were dirty and rooms inconsistent. Food was low quality-she was barely fed and was given frozen chicken nuggets she couldn't chew-and care felt insufficient: hospital transfer was denied, communication about her status was poor, and my mom later died; I believe an oxycodone overdose was involved. Staff will try to help and are responsive, but given the care failures and my financial concerns I am requesting a refund of the monthly fee.
