    Mixed care but hopeful improvements

My stay was mixed. Nurses and therapists (Audrey and others) were caring, responsive, and helped me recover - the rehab, activities, and ongoing remodels feel like real improvements. But care is inconsistent: I saw unclean rooms, odor issues, delayed/incorrect meds, and inattentive or rude aides at times. I'm cautiously optimistic with new ownership, but would inspect the unit carefully before trusting long-term or end-of-life care.

    About Bay Harbor Post Acute

Bay Harbor Post Acute is a skilled nursing and rehabilitation center that provides a mix of post-acute care, long-term care, and respite care in a comfortable and inviting setting, and you'll usually find Rose O'Neill, the Director of Community Relations, working to keep families and the community informed and involved, which is probably why they have things like the Family Matters program, monthly calls from the medical director for updates and questions, and a 24-hour hospitality line for residents and families who need help or just want to talk about care. The staff offers a broad range of services, so whether someone needs physical therapy, occupational therapy, or speech therapy, they handle that right on site up to seven days a week, and there's a team approach for complex care needs like wound care, cardiac care, pulmonary care, memory issues, diabetes, post-surgical recovery, and orthopedic rehab, and if a resident needs help with pain management, IV therapy, or nutritional support, those services are available too.

The facility runs a robust rehabilitation program, using multi-disciplined therapy plans that match the skilled needs of each resident, and the focus always stays on restoring as much independence as possible. They also have unique programs like Urgent SNF™ for fast stabilization and skilled nursing supervision when a quicker return from the hospital is needed, and Care Navigation, which helps people return home smoothly when they're ready, plus bariatric care programs and memory and dementia care for those who need it. If a caregiver at home needs a break, Bay Harbor Post Acute offers respite care too, and for residents nearing the end of life, there's hospice and palliative care designed to provide comfort and support.

You'll notice the place puts a lot of attention into guest experience, with amenities like beautician and barber services, the LifeLoop technology to help residents connect and keep up with activities, life enrichment programs for all functional levels, scheduled visitation, and virtual support through their life enrichment teams, along with a dining service offering a variety of meal choices to fit all sorts of taste preferences. They've also designed their infection safety approach with guidance from infectious disease specialists, which means residents get care in a safe environment, and there's access to a variety of physicians, plus psychiatry and psychotherapy services right on site. The building itself has a warm, homelike feel with comfortable lounges and activity spaces, and the staff aims to make recovery feel less like staying in a hospital and more like a supportive step towards going home.

Bay Harbor Post Acute carries a full list of healthcare and support services, from subacute rehabilitation and urgent skilled nursing, to restorative therapy, wound care, and long-term support, with a trained team that keeps families in the loop with wellness reviews every quarter and scheduled updates.
