Buena Vida Continuing Care & Rehab Center sits in Brooklyn, NY, right in Kings County, with the zip code 11221, and tries hard to create a calm and welcoming place for people who need short-term rehab or long-term nursing care, and you'll find that they focus a lot on safety, too, because they do tests like COVID-19 antigen and glucose testing, and they also have new rehab equipment in their updated gym for physical therapy, so residents have good options whether they're working on recovery or staying longer term. Dr. Peyman E Younesi serves as the director, while Phillip Rosenberg stands as the primary contact for the center, and they both seem focused on supporting residents' health and comfort, and, you know, there's actually a Diabetes Program set up to help manage diabetes and a Memory Care service for folks with Alzheimer's or dementia, plus there's even a Spanish Program for Spanish-speaking residents, and everything's made a little easier by the residents' concierge service that really helps with tours and questions. The center offers lots of recreational activities and everyday amenities, and people remark on the clean facilities and the warm, gentle care the staff provides. If you can't get there in person right away, they've arranged virtual reality tours so you can see the place from home, and their brochures come in English and Spanish, making information easy to get. Buena Vida's committed to keeping people safe with their COVID-19 steps and tries to make sure everyone feels as comfortable as possible, whether they're staying short-term for rehab, getting help with memory care, or needing support to live as independently as they're able. The center holds a waiver certificate, runs under CLIA number 33D0983159, and goes by facility ID N715-0000, which lasts until June 7, 2026, so everything's in order and checked, showing attention to doing things the proper way.

Clean facility, caring staff, management
I have a mixed impression. The facility is clean, with a good rehab center, engaging activities and many caring, knowledgeable day staff - my loved one, even while oxygen-dependent, was comfortable and trusted the 24-hour care. But management is often unreachable, communication and follow-up are poor, there have been medication/discharge problems, missing clothing, inconsistent housekeeping and some unprofessional or rude staff (including an alleged physical incident). I wish management would do undercover inspections and fix these lapses.
